eBenefits Confirmation Statements A Confirmation Statement is received within 24-48 hours of submission If the employee submits their benefits at 9 pm, the Confirmation Statement will not arrive until close to the 48 hour mark Confirmation Statements are not received on Saturday/Sunday – any submissions during this timeframe will receive their Confirmation Statement on Monday *Note: If an error occurs during the finalization process (ie. Due to missed DOB, Phone Number, etc) the enrollment will not process to the vendor(s), however; a Confirmation Statement will still be received within the 24-48 timeframe. State Group Health Insurance Student Health Insurance Plan (SHIP) International Students and J-1 Scholars are required to purchase the Student Health Insurance Plan or file a qualifying waiver. International Students are responsible to work with SHIP office and International Faculty & Staff Services (IFSS) teams to ensure they satisfy all Visa requirements. Flex Spending Account (FSA) with TASC What is an “FSA” and when can a new hire enroll? An FSA allows employee to set money aside from their paychecks pre-tax that they then may spend on certain eligible health or childcare expenses throughout their plan year. Plan options: Medical/Health FSA Dependent Daycare FSA New Hire enrollment(s) must be done within first 30 days based on date of hire. *Note: Fellows, Scholars, Graduate Interns/Trainees, and Post-Doctoral Fellows/Trainees are not eligible to enroll Flexible Spending Account(s) The Flexible Spending Accounts allow you to set money aside pretax that you were planning to use on medical or childcare related expenses for yourself or your eligible tax dependents. 2017 UW-Madison Benefits Services

2017 UW-Madison Benefits Services Earnings Statements Employees must review their Earnings Statements: Rate of pay & hours worked Insurance premium deductions Premiums are paid one month in advance Example: 9M paid on 09/01/17 pays for September’s coverage New hires will often require catch-up deductions depending on effective dates of coverage *IMPORTANT* Employees should contact Chrystin Schultz– your Payroll & Benefit Coordinator - if something is missing or incorrect 2017 UW-Madison Benefits Services
