Important points to note – Claim Submission Intimation of Hospitalization to UHC Customer Service within 7 days from Date of Admission The claim needs to be submitted within 30 days from date of discharge else the claim is liable to be rejected, following important points to be noted.. Claim Form Completely filled and signed Discharge Card & Main Hospital Bill: Discharge Card & Main Hospital bill should have the sign and seal of the hospital. In case of Credit hospitalizations, the Insured / Patient’s signature is mandatory on the discharge card and main hospital bill when they are discharged. Non Relevant Expenses: Document Recovery (DR) will not be raised for non-mandatory documents which are Irrelevant and not related to the present ailment. The amount of such expenses will be deducted from claim amount and closed at the very first instance itself. Service Charges Break-up: In case of Absence of Service Fees / Surcharge Break up in Main Bill the same will be deducted and closed.
Documentation Required for Hospitalization claims HIV and Related Investigation Expenses : Will be deducted from all claims by default - HIV forms part of General Exclusion Under a Mediclaim policy which cannot be waived off even after payment of Additional Premium (Non-Waiver Exclusion). This Investigation does not form part of the treatment, although performed as per hospital protocol. Non Mandatory Documents Valued below Rs.200/- : Will be deducted and closed with out Document Recovery Process. Advance / Deposit Receipts: These are mandatory documents without which the claim cannot be filed.
