Practice manager/nurse feedback Practice manager to meet with Louise Egan, lead manager for District Nurses Practice nurses report lack of communication and feedback as key issue. Lack of communication to patients also an issue
How we commission District Nurses Registered patient list Named Nurse for each practice Share of community matron – according to numbers on VP list
How we plan to drive improvements Focus on devising and implementing care plan – away from tasks Able to assess patients when unwell and appropriate response Key team member at MDT meetings Communication and call time standards
