Community Nursing Update CCF Community Nursing Update Thursday 3rd April

Issue 1 – attending meetings Majority of practices have issues with District Nurses not attending meetings When nurses do attend – feedback that often do not know the patients

Issue 2 – Home visitis District Nurses often unable to undertake home visits Specific feedback that district nurses have left before patients can answer the door

Issue 3 – Undertaking key tasks Example of District Nurse not complying to provide incontinence pads Refusal to provide injections to a patient on a Saturday

Practice manager/nurse feedback Practice manager to meet with Louise Egan, lead manager for District Nurses Practice nurses report lack of communication and feedback as key issue. Lack of communication to patients also an issue

How we commission District Nurses Registered patient list Named Nurse for each practice Share of community matron – according to numbers on VP list

How we plan to drive improvements Focus on devising and implementing care plan – away from tasks Able to assess patients when unwell and appropriate response Key team member at MDT meetings Communication and call time standards
