COMMUNITY PARTNERS PROGRAMME CPP Federal Government Programme FOR ELDERLY PEOPLE FROM ETHNIC COMMUNITIES

THE COMMUNITY PARTNERS PROGRAMME (CPP) As we get older we may choose to get support in the home or in the community. Later we may want to live in an aged care facility. Organisations that provide support to the elderly are called “aged care support service providers”.

WHAT DOES THE CPP WORKER DO? The CPP worker is the link between the aged care services and the Serbian community We tell aged care services about the Serbian community And we tell the Serbian community about aged care services Aged Care Service Providers CPP Nelke and Julia Serbian community

WHY? SO THAT ELDERLY SERBIAN PEOPLE HAVE GREATER ACCESS TO CULTURALLY APPROPRIATE AGED CARE SUPPORT SERVICES HOW: The CPP worker gives information to the Serbian community on aged care services so you can make the right decisions AND The CPP worker teaches Aged care services about the Serbian community and their needs so they understand you better

- Meet with Aged Care Support Services in the Illawarra More aged care support services delivering culturally appropriate care to older Serbian people HOW? CPP workers to: - Meet with Aged Care Support Services in the Illawarra - Provide Serbian cultural information kits - Teach Aged Care Services on Serbian culture and the needs of elderly Serbian people

INFORMATION MEETINGS - Once every quarter - Guest speaker from Aged Care Service provider will visit - Interpreter - Written information will be in your language and English
