Table Talk What are you struggling with in your own facilities? What do you hope to learn from one and other? If you did a site visit or have other experience with different facility types what do you notice?

Network Mapping We are collecting information to draw a beginning map of our network. We know from research about change that understanding our network helps us build effective relationships and solve tough problems. Network maps have been powerful tools in other collaboratives such as the Pittsburgh VA and Regenstreif Institute work on MRSA. So, please take just a few minutes to fill out our questionnaire.

spreads really matter need to maintain If you want to me interesting, you better be interested … if your bored … guess what even contagious … good ideas take hold … good ideas with passion soar The Most Valuable People in Your Network ~ Rob Cross U VA … The secret to these networks has never been their size … quality. Dunbar rule of 150 says … he who seeks to change the existing order … lukewarm supports … unless they are freinds Build relationships before you need them and ask them to help you … social capital … not exposure. 3. Brand your passions … focus on boundaries and concentrate your efforts … be a magnet

PLEASE DO Each person should do the following Make sure your name is on the list – if not add it. Fill out the first page by answering the questions For each name on the list: In the first column, put a check mark next to the names of people that you HAVE worked with in the past on infection control In the next column, put a check mark next to the names of people that you WANT to work with on infection control A TIP: Start with your facility, then your cluster, and go from there. Return your form to us today!

Planning for Improvement

What is “the work” you are planning? How will your cluster work together How will you do work in your own organizations? Who is on your team Who else should be? How will you share what you learned today with the rest of your team? How will you work together Scheduled meetings Huddles

The Model for Improvement Study Do Plan Act Increase appropriate use of contact precautions How will we know that a change is an improvement? What changes can we make that will result in improvement? Aims Measures Changes W.Edwards Deming *2001 Associates in Process Improvement

Collaborative Measurement Strategy SNF team list includes measurement contact Will schedule a call ASAP for hospital and SNF measurement representatives Briefly Nora McElroy, MA DPH Epimiologist CDI measurement Based on NHSN Lab-ID definitions and algorithms Measurement Graphic in handouts and Toolkit Hospital admissions and transfers for residents with CDI or consequences of CDI

Cluster Planning Sit with your cluster Pull your chairs into a circle away from the table if you need to fit everyone Everyone: How would you like to work together? Each facility: Vhat would you like to work on first? Volunteer? Pick one idea and try to fill out a PDSA worksheet

Collaborative Practice Opportunities Table

Planning your Changes (in your handouts and toolkit)

Cluster Planning We’ll regroup at 4:05 Stay seated with your cluster

spreads really matter need to maintain If you want to me interesting, you better be interested … if your bored … guess what even contagious … good ideas take hold … good ideas with passion soar The Most Valuable People in Your Network ~ Rob Cross U VA … The secret to these networks has never been their size … quality. Brand your passions … focus on boundaries and concentrate your efforts … be a magnet

To Sustain or Re-ignite Momentum Find your passion! Tell your friends at work! Expand your network: tell someone new what you’re doing! Ask them a question!
