Primary Tasks of ASU New Licensure for CCLC FCCLH Licensing Changes of Ownership Change of Locations (CCLC and FCCLH) All initial CCLC/GDCH applicants MUST attend the Licensure Orientation Meeting. The LOM certificate expires two years from the date of attendance. FDCH applicants must attend an ROM and the ROM certificates do not expire. ©2007 Bright from the Start

Licensure Application Process Starting Point: Attend LOM training Submit Application Part A along w/ Floor, Site and Operation Plans w/ checklists (Acknowledgment letter will be e-mailed to you) Assigned to & Reviewed by (Proceed or repeat) ASU Consultant Submit Part B Application and other documentation Review Rule Checklist via phone; Schedule Initial Visit Conduct Initial Licensing Study on-site (ILS) Begin operating as a Licensed Facility* & pay fee within 30 days Submit Annual License renewal fee As you have already attended the LOM, this is just a review of the process and time lines for applications. Please note it is not realistic for a new facility to become licensed in a month. -Discuss new construction- we encourage the applicant to go ahead and submit part A. This will allow time for any needed modifications during building. With existing buildings, the applicant is responsible for bringing the facility up to the most recent licensing rules and regulations prior to being granted PTO. -Plans reviewed on first come / first received basis. *NOTE: You are not allowed to provide care until PTO (permission-to-operate) is granted. Part A Requirements Cont. Notarized CRC application for director, if director is listed on application Complete, readable floor plan w/ completed FP checklist Complete, readable site plan q/ completed site plan checklist Operation plan materials w/operation plan checklist Large stamped, self addressed envelope Sonya reviews the packet to ensure the above items are present. If all of the items are present, the application is then assigned to an ASU consultant who then conducts the in-depth review of all of the items. ©2007 Bright from the Start

Part B Requirements: Completed Part B application Completed vehicle inspection State Fire Marshall’s report showing recommendation for a CO Building Inspector’s report or CO Results of CRC check on director AND ALL hired staff Confirmation of city water/sewer or water/septic tank approval and capacity ©2007 Bright from the Start

Part B Cont. If food is catered: food permit & current health inspection with graded score Copy of Director’s Education qualifications 40 hour director’s training certificate Completed ILS Staff Profile form ©2007 Bright from the Start

FCCLH Application Process Starting Point: Attend FCLH LOM training Submit Registration Application and all other required documentation Reviewed in Atlanta office then assigned to ASU Consultant if Approved Emailed and/or contacted by phone by ASU Consultant to review for visit ILS Visit scheduled Conduct Initial Licensing Study on-site (ILS) Submit License fee & begin operating as a Licensed Facility Submit License renewal fee The process is similar for FDCH applicants. FCCLH Application Submission Checklist Completed FDCH Rules and Regulations Checklist Complete Application Notarized Citizenship Affidavit with Verifiable Documentation ROM Training Certificate (Does NOT expire) Copy of First Aid/CPR cards Training Certificates documenting ten Pre-Service training hours Verification of Education Credential & Official Transcripts Disclosure forms Part I and II Zoning Letter Lease Agreement if applicable Business License Floor Plan Template These are the required items for the submission of an FDCH application.
