Objective To comprehend and apply factors and procedure in the preparation of accident incident report

Introduction Any particular event, activity or Opns in military service has a certain built-in probability for accident or Incident to occur. Accident are like thieves in the night we might never know when they strikes.

DEFINING ACCIDENT/INCIDENT Mishap - Misfortune involving aircraft / vehicle properties and person that does damage to both Accident - A unforeseen event that could cause injury to PAF personnel and or damaged to property - An unfortunate occurrence implying some degree of substantial damage, fatality or injury. Incident - An occurrence wherein the degree of damage is minor and lesser magnitude

DEFINING HAZARDS Hazard - Any event or situation that does no damage or injury, but clearly has a potential damage or injury to do. - An accident or Incident waiting to happen Operational Hazard - Condition or act of person that may affects the safety of a/c, veh, equipment, and other government facilities to include our personnel Hazard reporting is a step towards an acc/inc prevention

OHRS It is a venue or way of reporting situation that could affect the safety of PAF units. It contains basic information like location of hazard, type of opn flight or ground and brief description. Upon receipt, a safety officer/NCO will: - Verify the existing of hazard - Analyze the danger it can pose - Panel route & provide info to proper channel for investigation & resolution of the fact

THE NEED TO REPORT Source ; Safety Management Handbook There is a need to report Acc/Inc and even Hazard in order to inform the head of office of that event and to pave the way for an investigation TO DETERMINE the cause and to PREVENT recurrences. Reporting may eventually lead to accident and incident prevention. Hazard reporting aims to correct, not to reprove

TYPES OF REPORT Flash Report - Any means of giving informal report for the purpose of immediate information to higher headquarters.

TYPES OF REPORT Preliminary Reports - Progress report having detailed information of the mishap submitted w/in 8 hours after the Accident/Incident

TYPES OF REPORT Accident/incident report - After accident / incident report of the persons concerned from the start of the mission up to his recovery phase. This is the vital part of the formal report to assess what had happened in the occurrence.

TYPES OF REPORT Formal Report - Detailed record of the investigation of an Accident / Incident and must be forwarded to the CG, PAF w/in 30 days

COMPOSITION OF THE BOARD Chairman Investigating Officer Asst Investigating Officer Maintenance Officer for aircraft-related acc/inc Medical Officer (if necessary ) Other essential personnel as designated Member/Recorder

PROCEDURES A. GET THE BIG PICTURE FIRST What property was damaged? How many people were injured or killed? What were the causes?

PROCEDURES B. INTERVIEW THE MOST KNOWLEGEABLE PEOPLE Interview in the spot. Interview should be in private. Put the person at ease. Get the individuals version. Ask the right person at the right time. Repeat his story once you heard it. End each interview with a positive note. Record information quickly.

PROCEDURES C. PREPARE THE REPORT Collate info gathered. Describe the incident. Make cause and effect of the facts gathered. Evaluate problems encountered Find the solution/conclusion Make recommendation's)/action plan to prevent or control recurrence Review the report. Finalize the report.
