DSRIP Updates Midpoint Assessment Midpoint Assessment will be initiated after August 31, 2016 The Mid-Point Assessment is a required component of the DSRIP Program   The Mid-Point Assessment will focus on the progress made by the NYP/Queens PPS through the end of the first quarter of DY2 towards establishing the necessary organizational foundation and towards the implementation of the project requirements consistent with the approved DSRIP Project Plan. The primary data sources used in the Mid-Point Assessment will be: Approved DSRIP Project Plan Applications Quarterly Report Submissions - DY1, Q1 through DY2, Q1 Claims and non-claims data for P4R/P4P measures 360 review with partners- DOH NYPQ PPS PAC Meeting

DSRIP Updates Patient Engagement and Update of Partner Form After reporting DY1 Q4 data to the NYSDOH, the NYP/Q PPS has received feedback from the IA that the documentation submitted by NYP/Q PPS contained Medicaid identification numbers which also appeared in the documentation submitted by multiple PPS. The NYP/Q PPS’ approach to resolving this duplication issue was to reach out to other PPSs and initiate a discussion to compare the rendering provider and their NPI for each identical patient shared. Solution: To decrease the likelihood of this reoccurring in future reporting, we plan to include the provider and NPI fields in our reporting templates or the partner form that are sent out to the sites every month Patient Engagement Data Reporting: It is very important to understand that same patient can not be reported as actively engaged patient from month to month for same project or measure NYPQ PPS PAC Meeting

DSRIP Updates Medical Record Review Between Aug 2016 and Feb 2017, the PMO team will work with the medical record collection team or the vendor abstraction services provided to us Locate and abstract data on randomly selected sample of patients to populate the measures we selected for DSRIP project After the kickoff meeting between vendor and PMO team by July 31, the abstraction partner or vendor will work directly with the providers to perform the sample pull and record reviews NYPQ PPS PAC Meeting

PPS Updates Communication Plan This Communications & Engagement Plan supports the NYP/Queens DSRIP PPS and is designed to enable effective communication and engagement of key stakeholders, network partners, patients, and workforces for effective functioning, contributing to success. This plan outlines the following plans Community & Engagement Plan Workforce Communication & Engagement Plan and Practitioner Communication & Engagement Plan and will be updated to include the Agency Coordination Plan Communication Methods: User guides, webinars, conference calls, bi- monthly committee meetings, newsletters etc. will serve as the primary channels to communicate progress and to present recommendations NYP/Q PPS PAC Meeting

PPS Updates Training Strategy Majority of the workforce funding by DSRIP goes into building and implementing an effective training strategy for staff, providers, CBOs, etc. Maximize efforts with existing programs and partnering CBOs A Survey will be conducted to determine the training capacity and training needs of each collaborator Collaborators will be encouraged to pursue their usual training procedures and abide by the Guiding Principles Collaborators lacking training resources will be invited to participate in quarterly training seminars hosted by the NYP PPS PMO and the Cultural Competence/Health Literacy Committee which will be live, half day events, accessible on-line; or they may participate in external trainings conducted by GNYHA or other institutions NYP/Q PPS PAC Meeting
