1. Parents and Young People involved in Crime and or Anti-Social Behaviour Indicators: A child under 18 years old who has committed a proven offence in the previous 12 months An adult or child who has received an anti-social behaviour intervention (or equivalent local measure) in the last 12 months An adult prisoner who is less than 12 months from his/her release date and will have parenting responsibilities on release An adult who is currently subject to licence or supervision in the community, following release from prison, and has parenting responsibilities An adult currently serving a community order or suspended sentence, who has parenting responsibilities Adults or children nominated by professionals because their potential crime problem or offending behaviour is of equivalent concern to the indicators above Significant and Sustained Progress: No offending in last 6 months by any member of family home or 30% reduction (sliding scale) No known ASB committed by members of the family home in the last 6 months or successful completion of an Acceptable Behaviour Intervention, Criminal Behaviour Order (CBO), Crime behaviour injunction, housing injunction or other appropriate order Reduction in police call-outs to family home by 60% for six months as compared to previous 6 months Croydon Strategic Goal: Safer Croydon Community Safety Strategy 2014-17: Reduce the overall crime rate in Croydon (focus on violent crime) / Improve the safety of children and young people / Tackle anti-social behaviour and environmental crime

2. Children who have not been attending school regularly Indicators: A child who is persistently absent (currently measured as missing 15% of sessions) from school for an average across last 3 consecutive terms [Note: this will change to 10% in 2015 as DfE threshold changing to 10%] A child who has received at least 3 fixed term exclusions in the last consecutive 3 terms A child at primary school who has had at least 5 school days of fixed term exclusions in the last consecutive 3 terms A child of any age who has had at least 10 days of fixed term exclusion in the last 3 consecutive terms A child who has been permanently excluded from school within the last 3 school terms A child who is in alternative educational provision for children with behavioural problems A child who is neither registered with a school, nor being educated in an alternative setting A child nominated by education professionals as having school attendance problems of equivalent concern to the indicators above because he/she is not receiving a suitable full time education Significant and Sustained Progress: Every child in the household has attended school in excess of 90%, over three consecutive terms, with no more than 2 fixed term exclusions per person and no permanent exclusions Primary school pupils have no more than 3 days of exclusion in 3 consecutive terms Alternative education: improvement in attendance against base-line figure and or improvement in behaviour – key worker discretion/ local measures e.g. PRU local target on attendance (Possible: children who are not registered with a school become registered) Croydon Strategic Goal: LBC Children and Families Plan 2015-2016: 4. 3. Children who need help Indicators: Children who don't take up the Early Years Entitlement (a child who has been identified as needing early help) Children identified in the School Census as having social, emotional and mental health problems (a child who has been identified as needing early help) Children who have been reported as missing from home and identified as of concern (a child who has been identified as needing early help) Children who are repeatedly assessed under Section 17 or 47, of the Children Act 1989, but not deemed a 'child in need' (a child who has been identified as needing early help) Children who are subject to Early Help Assessments (a child who has been identified as needing early help) A child 'in need' under Section 17, of the Children Act 1989 A child who has been subject to an enquiry under Section 47, Children Act 1989 A child subject to a Child Protection Plan A child nominated by professionals as having problems of equivalent concern to the indicators above Significant and Sustained Progress: Early help referred case closed and there are no repeat referrals in the following 6 month period Improved behaviour of a child or young person for a 6 month period and improved parenting skills where relevant, including mental health. Measured by families outcome star (or equivalent local tool) Improved family functioning for a 6 month period (improved ability to manage conflict in the family and in family relationships) over a 6 month period. Adults out of work or at risk of financial exclusion, & young people at risk of worklessness Indicators: An adult in receipt of out of work benefits An adult who is claiming Universal Credit and is subject to work related conditions A child who is about to leave school, has no / few qualifications and no planned education, training or employment A young person who is not in education, training or employment Parents and families nominated by professionals as being at significant risk of financial exclusion. This may include those with problematic / unmanageable levels and forms of debt and those with significant rent arrears Significant and Sustained Progress: 13 weeks consecutive employment (or 26 out of last 30 weeks for JSA) An adult or over 16 (NEET) makes job ready progress to work; enrolled in higher or further education or apprenticeship for at least 13 weeks (or completion of whole training course if lasts between 8-12 weeks), or formal volunteering or work experience or a programme or course which removes a barrier to work for the individual An income and debt re-payment plan is in place and implemented for at least 13 weeks and there is no escalation in sanctions Remains on universal credit but meets earnings threshold (£330 for adults 25+ or £370 for >25) Ending fuel poverty - family do not have to spend more than 10% of their household income on fuel to keep their home in a satisfactory condition, 6 months Securing and or, maintaining suitable accommodation that is; affordable, dry, well maintained and safe from violence or threats of violence and reasonable in comparison to general housing circumstances in the area (suitable accommodation secured/ maintained for at least 6 months) – Overlap with HEALTH outcome Croydon Strategic Goal: LBC Children and Families Plan 2015-2016: 7. 5. Families affected by domestic violence and abuse Indicators: A young person or adult known to local services has experienced, is currently experiencing or is at risk of experiencing domestic violence and abuse A young person or adult who is known to local services as having perpetrated an incident of domestic violence or abuse in the last 12 months The household or a family member has been subject to a police call out for at least one domestic incident in the last 12 months Significant and Sustained Progress: Victim satisfied with support received through police/ criminal justice process or targeted support provided Increase in safety, demonstrated by locally agreed distanced travelled tool Perpetrator engages/ accepts support and no further assessment required for 6 months Croydon Strategic Goal: Croydon’s Domestic Abuse and Sexual Violence Strategy 2015: Prevention/ Protection and reducing the risk/ Provision of support to reduce violence against women and girls/ Partnership

6. Parents and children with a range of health problems Indicators: An adult with mental health problems who has parenting responsibilities A child with mental health problems An adult with a drug and / or alcohol problem who has parenting responsibilities A child with a drug and / or alcohol problem A new mother who has a mental health or substance misuse problem and other health factors associated with poor parenting. This could include mothers who are receiving a Universal Partnership Plus service or participating in a Family Nurse Partnership Adults with parenting responsibilities or children who are nominated by health professionals as having any mental and physical health problems of equivalent concern to the indicators above. This may include unhealthy behaviours, resulting in problems like obesity, malnutrition or diabetes Significant and Sustained Progress: Parent takes responsibility for managing their family’s health demonstrated using all or some of the following measures when applicable: Care plan or self-care strategy in place where there wasn’t one before, to be maintained for 6 months All children in the household have received age appropriate vaccinations Take up of dentists services (registration, check-up for each child/ adult of the family in the last 6 months) Reduction in weight to a healthy level (BMI rating) of one household member who has been assessed as overweight or obese, to be maintained for 6 months Making regular use of effective birth control methods YP is well informed and takes responsibility for their health for 6 months Good level of development in Early Years demonstrated by family outcome outcomes star Increased feeling of well- being using locally agreed distance travelled tool 33% reduction in hospital admissions (12 month baseline, performance measured on 6 months data doubled) 33% reduction in attendance at A&E (12 month baseline, performance measured on 6 months data doubled) Family member reduces intake and harm in use of drugs or alcohol over 6 months (inc. child smoking cessation) Securing and or, maintaining suitable accommodation that is; affordable, dry, well maintained and safe from violence or threats of violence and reasonable in comparison to general housing circumstances in the area (suitable accommodation secured/ maintained for at least 6 months) Family demonstrate reduced social isolation by participation in a positive activity for 8 weeks or more e.g. parenting course Croydon Strategic Goal (if known): LBC Children and Families Plan 2015-2016: 1.
