the BroncoSERV Database: Faculty Guide

WHAT IS BroncoSERVE? BroncoSERV is an online database that manages approved service-learning placement sites, tracks students’ placements and stores required forms for students’ service-learning activities. The forms students complete on BroncoSERV student forms can be accessed by faculty. The database currently lists all approved service-learning sites, uploadable service-learning agreements and site visit documentation.

INFORMATION ABOUT STUDENT PLACEMENT FORMS Release of Liability Form states that the student is voluntarily participating in the service-learning activity and does agree to hold the University harmless from any injury, illness, disability, economic or death claims. A signature from the student states that they understand the legal consequences of signing the document, that they release the University from all liability, that they promise not to sue the University and that they assume all risks in participating in the service-learning activity, including travel to, from and during the activity. They must sign this form prior to service. Learning Plan and Participation Form gives students the opportunity to think clearly about the ways that the service-learning placement connects with, and reinforces in-class learning. The plan contains specific segments: verification of placement, definitions of time and schedule of service, participation guidelines to outline appropriate behavior expectations. The student’s signature is required prior to beginning service to demonstrate that they are aware of the potential risks involved and expectations of the service-learning experience. Emergency Contact Information provides student’s information, site information and emergency contacts. It also provides a comments section for allergies or health considerations. The form provides an opportunity for faculty, site staff and CPP designated staff and administration to have contact information should an incident or injury occur. A signature on this form indicates that the student verifies that the information they have entered is correct. They must sign this form prior to service. Video/Audio Image Release Form A signature on this form allows Cal Poly Pomona, the Center for Community Engagement and your faculty member to take and use visual and/or audio images of the student. Students have the option of declining that their images be used and it does not prohibit them from being at a placement site.

LogGING into BroncoSERVE To log into the BroncoSERV database, visit https://app.calstates4.com/cpp and follow the steps below: Click on Log In Button (Green Button in Right Corner) Choose Cal Poly Pomona BroncoSERV from drop down menu that comes up on next page Enter your CPP username and password to access the site

VIEWING YOUR BroncoSERV HomePAGE Your name should come up on your homepage on the screen as Hello ____________! Click on Manage Your Courses link

SelectING SITES for Your STUDENTs Your specific course should appear on this screen. To select sites for your students’ service, click on the green Select Sites button.

SELECTING SPECIFIC SITES FROM The LISTING To select specific sites from the listing, click on the green bars with the organization’s name on it. This will limit your students to sign up only at these sites. Once you are done selecting, click on I’m done with this course. Complete process. Please note if your students can choose between any of the 100+ sites, click on I’m done with this course. Complete Process.

vIEWING YOUR COURSE You have access to see if your enrolled students have completed their required forms as well as to the forms themselves. You can click on the course name or the View Course button.

ACCESSING STUDENT FORMS The entire student listing of your course comes up and you can easily check under the Forms? Section to see a Yes or No indicating the student has completed the forms. For a pdf of the forms, click on the Student Forms tab next to the View tab.

ACCESSING STUDENT FORMS All the completed forms appear by student’s name. You can click on them and will get a pdf of the form the student signed.
