Problem/Opportunity Statement Sepsis Project Charter Template Problem/Opportunity Statement There are 1 million hospital admissions per year for sepsis in the US. Sepsis accounts for: ~20% of all ICU admissions Up to 50% of all ICU deaths More annual deaths than from heart attacks More annual deaths than from breast cancer, prostate cancer and AIDS combined Highest cost admission diagnosis Hospitalizations per 100,000 discharges has steadily increased every year from 126(year 2003) to 342(year 2011). Beginning Oct 2015, all hospitals will report their sepsis performance to CMS. Beginning in Oct 2016, payment to hospitals will be adjusted based performance on a new sepsis quality measure(SEP). Implementing HVHC’s evidence based 3-Hour bundle has been associated with reductions in mortality and costs by up to 50%. Project Aim Implement the HVHC 3-Hour bundle at all participating units.
