What you need for the DMV Permit Exam Receipt Social Security Card – Satisfies Group A & C Identification Proof of Insurance Completed 50-hour Driving Sheet Certified Copy of your Birth Certificate – Satisfies Group B Identification $20 (either cash or Check made out to the “Secretary of State”) Proof of Residency (school Transcript, bill with your name on it – need two sources needed; Satisfies Group D) Car to drive – Make sure all lights and systems are functional (and that you know how to use them) because they will be checked there If you have an “A” or “B” in class and “A” or “B” in Behind-the-Wheel you will be eligible for the cooperative driving test. If you pass that test, you will need to bring that form as well. (Not every test at the end of the term is the cooperative test. Speak to your driving instructor about this if you are not sure).
