The most pressing issue should be rated on each occasion. If 0-no problem is selected, no letter is required. If J- Other is chosen, an indication of the nature of the problem is needed. Clustering Best Practice 2017 At Discharge….. At discharge, the cluster episode should be close by creating a new record. Complete the clustering assessment to complete outcomes information (HoNOS). If the client hasn’t been seen recently, then unknown or 9 can be recorded. Select Discharge from the clustering allocation drop down menu. Complete the last fields “Discharging from clustering”. The date should be consistent throughout the form Don’t edit old records CW/LVW – reporting outcomes
