Berlin Area School District 1633 Students in a rural setting 55% Free and Reduced Lunch Highly Transient Population (30% Transfer into and out of district in a typical year) 1 Elementary, 1 Middle, 1 High School 50 students identified as Homeless for the 2017-18 School Year (3%)

The Journey Into Tier III Has Just Begun Started in the fall of 2016 Assembled two systems teams at the elementary and middle schools Trained 3 facilitators (one of whom recently retired) Created concrete decision rules for each school for eligibility Collaborated with the County Coordinated Service Teams to avoid service duplication Engaged 2 families in our first year (1 at elementary and 1 at middle)

Elementary Example 1st grade student Decision Rules for Inclusion: 29 ODRs through December 2016 Counselor Referral Lack of Success on Tier II Interventions (CICO & SAIG) Only 7 ODRs for the remainder of the school year after Wraparound Services began

Middle School Example 8th grade student who moved into the district Family was housing insecure with legal and child and family services involvement Decision Rules for Inclusion: Multiple behavioral health inpatient hospitalizations for the student Counselor referral County Coordinated Service Team program was full

The Principles in Action Family Voice and Choice & Collaboration “I like how you really listen to our issues and let us come up with ideas to solve them instead of telling us what to do.” – Wrap Family We meet at dates/times/locations of the family’s choosing. We discuss the topics and issues that are important to the family. All solutions are considered, none are discounted. Due to the student’s significant mental health concerns, we regularly collaborate with outside mental health service providers as requested by the family to coordinate care.

The Principles in Action Team Based & Natural Supports I like how you let me bring my friend to the meeting. No one has ever asked me who I wanted to come to these types of meetings before. – Student Participating in Wrap Student was allowed to bring a friend who they felt was a true support in their mental wellness. Friend brought significant insight into the discussions.

The Principles in Action Strengths-Based and Individualized I like how you focus on the positive things in our family. I don’t really think about the things we do well, I usually just think about our problems. – Wrap Family One really successful plan around family unity was leveraging the family’s strength in using humor and fun, by instituting a regular movie night once per month. As the student transitions to the High School setting, we are working on independent skills so the team decided the student should coordinate scheduling the meetings and reminding the team to attend. Highly successful.

The Principles in Action Outcome Based The student has a better affect and mood in school. I’ve seen them smile for the first time. They seem to have more friends and engage in fewer behavior incidence. –MS Principal One hospitalization since Wrap began in March but only due to inability to manage medications through Outpatient Behavioral Health services. Student created schedule for High School and now expresses greater hope for the future. Student’s grades and time in class have significantly improved

Growing Pains As this student transitions to High School the facilitator will continue to meet with the family despite High School not yet at Tier III fidelity. The district is now down 1 facilitator (due to retirement) and had planned to be at 4 facilitators to facilitate 10 Wraps for the 2017-18 school year. Maintaining a high level of fidelity at Tiers I & II as we grow our Tier III offerings.
