Building an Asthma Registry Welcome to the UPIQ/Asthma Program Learning Collaborative Building an Asthma Registry Gabi Baraghoshi RN MPH – QI Coach

Why building a registry? Patient care registries are moving more and more into the mainstream thinking as a “must have” resource for clinicians Population management with registries are foundational in the patient centered medical home model and are important for delivering better quality care A registry can help identify pts who have left the practice. Pts who have not been in contact with the practice for over 12 months should be contacted to determine whether they are overdue or whether they have left the practice.

What Information Goes into a Registry? It needs to include both dates and values for crucial intervention (overdue for FU visit, lab tests, …..) Asthma Registry (minimum): Demographics Date of last visit Severity and control levels Date of AAP Date of FU visit Date of last flu shot

Where does the Data come from? There are 3 ways to enter data into a registry: Manual data entry into an excel spreadsheet Download from an EMR Registry build into an EMR (needs to be more than just a pt list)
