DISASTER RESPONSE TEAM IN HOSPITAL RESPONSE AFTER EARTHQUAKE 2072 Dr padam khadka,Vice president APHIN Western region

disaster Disaster scenarios once seemed merely theoretical now becoming a disturbing reality. For natural disasters like earthquake, there is no any advance warning, as of now. Hospitals- first institutions to be affected after a disaster. So they need to be prepared to handle such an unusual workload.

Disaster management D I S A T E R DISASTER INDUCED SITUATION AFTER SEVERE TRANSFORMATION OF ECOLOGICAL RESPONSE

OUR MAIN AIMS USE LIMITED RESOURCES AVAILABLE IN A MODIFIED WAY TO SAVE AS MANY LIFE AS POSSIBLE. DELIVERING CARE TO THE LEVEL OF USUAL HOSPITAL STANDARDS OR BENCHMARKS MAY NOT BE POSSIBLE AND an ‘ALTERED STANDARD’ HAVE TO BE ACCEPTABLE. ALTERED STANDARD 1.mean-changing infection control standards to permit group isolation rather than single person isolation units 2.limiting the use of ventilators to surgical situations 3.creating alternate care sites in the waiting area, lobby or corridors which are not designed to provide medical care 4.It could also mean changing who provides various kinds of care like enhancing the scope of nurses, physician assistants and hospital paramedics.

Hospital disaster plan Hospital disaster plan is prepared to reduce the pressure on the hospital management when a large number of casualties arrive suddenly in the hospital at a time. The plan- activated immediately to provide efficient care to the pt. within a short span of time. Mock drill- conducted periodically Adequate storage of supplies in the emergency department.

Internal disaster plan Multidisciplinary approach- 1.safe working physical status of the hospital- alternate sites 2.adequate equipment 3.sufficient trained human resources 4.logistics- power supply with alternative backup, adequate reservations of water supply. 5. Proper transportations facilities 6. Pharmacy supply

External disaster plan Co-ordination and communication with other institutions. Documentation and record keeping Hospital traffic control Periodic public notifications Resource mobilization and collaboration with other teams.

WHAT SAVED US FROM SUCH A GREAT DISASTER..? IT’S THE CULTURE, HELPING ATTITUDE AND OUR SOCIAL NORMS AND VALUES. PROBABLY DUE TO THE GODS AND GODDESS OF THE KATHMANDU VALLEY. NOT DEFINITELY DUE THE GOVERNMENT’S DISASTER PREPARDNESS PLAN.

WILL THIS TYPE OF SOCIO-CULTURAL TO FACE THE FUTURE DISASTERS..? PRACTICE BE ENOUGH TO FACE THE FUTURE DISASTERS..? NOT AT ALL

WE DEFINETELY NEED TO HAVE AN EFFICIENT AND EFECTIVE NATIONAL DISASTER PREPAREDNESS PLAN…

LESSONS LEARNED… PROPER GOVERNMENT DISASTER PREPAREDNESS POLICY. DISASTER PRPAREDNESS PLAN FROM EVERY RESPONSIBLE INSTITUTIONS. PROPER CO-ORDINATION AND COMMUNICATION WITH THE STAKEHOLDERS.
