Access and Registration

Login to the Member Website Go to: HealthAdvocate.com/members Type in the name Type in Albuquerque Public Schools select it from the drop- down box and click “Submit”

Registration Process Click “Register”

Registration Process Enter your personal information as it appears on your paycheck, then click “Next”

Registration Process Create a unique User Name and Password, and provide your individual, active email address and choose your security questions (email account must be valid). If registering a spouse, a different email address must be entered For your security, your password must include at least six characters, and include at least one capital letter, one lower case letter, and one number Click “Next”

Registration Process Read and accept the Terms and Conditions, and click “Register”

Registration Process You will receive a message directing you to open the Welcome email and click on the verification link Welcome to Health Advocate

Registration Process Open your email account and the email from registration@HealthAdvocate.com Click on the hyperlink in the body of the email to activate your account

My To-Do’s: Incentive Information

My Points – Wellness Incentive Earn 50 points by completed the required items: Physician Engagement Form Tobacco Affidavit. Earn an additional 250 points throughout the program earn to be entered into prize drawings! Track your progress on the “My Points” section.

Complete the Physician Engagement Form Click on “Download” print the form, have your physician complete it and fax or email it back to Health Advocate.

Select and Complete the Tobacco Affidavit Click on “Complete Now”. Select the option that is most appropriate for your situation, check the confirmation box, and then click Submit. If you are a tobacco user , you will be required to complete the online tobacco cessation program to qualify for the incentive. Click Workshops and Programs.

My Points – Prize Drawing Incentive Earn an 75 points per quarter by completed additional wellness activities to be entered into prize drawings!
