Project ECHO Case Presentation Coaching Session Evan Klass, MD March 24th, 2016

WHY? Recent ECHO Nevada survey feedback indicated participants would like an actual training session for case presentations. Participants reported apprehension and anxiety about presenting ECHO cases during clinic (self-conscious in front of peers and specialists). ECHO thrives on case-based learning and ECHO Nevada falls short of target number of cases presented during each clinic (minimum of 2-3)

Learning Objectives Choosing the ideal case to present Introduce the process of finding, downloading, filling out, and sending ECHO Nevada Case Presentation Forms to the ECHO Nevada Team Approximate minimum and maximum data needed in order for ECHO Specialists to provide the most accurate and high-quality consultation for you. Benefits (for both ECHO Nevada and you) of submitting your Case Presentation Form at least 48 hours before the ECHO clinic.

Choosing the Ideal Case to Present Before you refer a patient to a specialist, ask yourself: “Is there an ECHO clinic for this disease?” If so, are you willing to treat your patient’s condition with the guidance of a specialist? If you answered yes to the first two questions, you have found a prime ECHO case to present. Our ECHO Specialists will also help you identify patients that truly need to be referred and they can help expedite their appointments when needed as well.

Finding the Case Presentation Form Select the applicable clinic from the “ECHO Clinics Page”: http://medicine.nevada.edu/echo/clinics

Downloading Case Presentation Form For this example, click “Diabetes Case Presentation Template”

Download or Print Form This is what the Case Presentation Form will look like:

Case presentation form

Submitting Your Case Presentation Form Currently, the only acceptable and HIPAA-compliant method we have is by fax. Fax number for ECHO Nevada: 775-327-5112 Only documents WITHOUT ANY Personal Health Information (PHI) can be emailed. Emailed documents must be completely deidentified.

Benefits for Submitting Your Case(s) Early! Cases will be discussed in the order they are received – this will be very important as we have more cases! Allows ECHO Specialists to request additional information if needed. Cases submitted before clinic will receive written recommendations from ECHO Faculty that can be added to patient’s medical records as a billable provider-to-provider consult.

Tips for Presenting Cases During ECHO Clinics Please do not use any PHI during your case presentation (i.e. name, DOB, or even job position if it can be used to identify them). ECHO clinics are a respectful and safe environment free of criticism and judgment – Our team is here to help you improve your level of specialty care through case consultations and co-managing complex patients.

Tips for Presenting Cases During ECHO Clinics (continued) As we begin to receive more cases each clinic, we will need to become more efficient with our time. Our goal is to keep each case to around 15 minutes total (5 min to present, 10 min for consult and Q&A). Remember, ANY level of provider can present a case during ECHO clinics and a single provider can present multiple cases for their facility – a presented case does not have to be their own patient.
