Context - Hospital ranked 2nd worst performing hospital in the UK - Unitary Authority need to make savings of £194 million in next 3 years - New Foundation trust created in April 2016 taking over all mental health and community hospitals - No Joint commissioning - Consistently in the top 5 for rough sleepers in the UK - Access to services due to geographical constraints

Cornwall’s model - Successfully bid to Homeless Hospital Discharge Fund 2013/14 - £149,000 - Partners included – RCHT, NHS Kernow, Shelter, Public Health, Coastline Housing, Inclusion Cornwall and CFMHT - 2 key aspects - Protocol and Awareness/Training Accommodation

Protocol & awareness/training - Originally 6 month project with further 2 month evaluation - Provide a protocol across both RCHT and Acute Mental Health Wards to facilitate safe discharge and reduce bed blocking - Provide training to hospital worker, homelessness officers, partner agencies to promote awareness of protocol - Hospital Homeless Patient Advisor - extended until the 31st March 2017

Accommodation - 6 units of accommodation for 2 years - Short term 6 week intervention / support to stabilise client - Co – ordinated support / care plan with move on
