IF THIS APPOINTMENT IS NOT KEPT YOUR SAMPLE CANNOT BE USED DONOR SPERM PATHWAY About 3 weeks after we receive your questionnaire you should attend an appointment in the Andrology department Read all information and then forward questionnaire to us if you wish to continue You will need to have a blood test for infectious diseases, urine test for sexually transmitted diseases (STD), and a semen analysis including a test freeze The results are then reviewed by a doctor. This could take 6 weeks. You will then be contacted for your next appointment, this may include seeing a counsellor. Further screening tests may be needed for cystic fibrosis carrier and genetic disorders The results are again reviewed by a doctor. This could take 6 weeks If you meet all our eligibility criteria you will be sent to Andrology to make 10-15 weekly appointments For sperm banking An appointment will be sent to you to discuss the results with a doctor. Your samples will be quarantined for 6 months and then we will send you an appointment for final blood tests.
