Accident Direct Superior Client/Broker Experience Changes in the market present opportunities to sell supplemental plans Essential Health Benefits Plans can have out of pocket expenses as high as $6,350 for individuals or $12,700 for families. This leaves large gaps in coverage which can be filled with supplemental plans. Advanced premium tax credits for EHB plans are available for individuals or families making up to 400% of the federal poverty level. Tax credits can be used to purchase extra coverage and fill the gaps left by high deductibles and coinsurance on EHB plans. Accident Direct Benefit Schedule Daily Benefits Description Plan 1 Plan 2 Plan 3 Plan 4 Accidental Injury Benefit Coverage for Accidental Injuries that first occur and result in a hospital confinement within 45 days of the Accidental Injury. Benefits refresh each plan year (plan year is each consecutive month period beginning with the effective date of coverage). Accidental Injury Benefit Payable for Hospital Confinement with or without Surgery: • 14 days or more: 100% of Accidental Injury Benefit Amount • 7 to 13 days: 60% of Accidental Injury Benefit Amount • 3 to 6 days: 30% of Accidental Injury Benefit Amount • 1 to 2 days with surgery: 15% of Accidental Injury Benefit Amount • 1 to 2 days without surgery: No benefit payable. $5,000 $15,000 $20,000 $25,000 Common Accidental Injury Benefit • Provides benefits in addition to the Accidental Injury Benefit • Limited to one Common Accidental Injury Benefit Amount under the policy per year • Pays 50% of the Accidental Injury Benefit Amount when 2 or more insured persons are injured in the same Accidental Injury and at least 2 of whom meet an of the criteria below: Criteria One: Hospital Confined for 3 or more days Criteria Two: Hospital Confined for 2 or more days with surgery

Accident Direct Claims Example Jenny Heart, age 43, had a car accident. Jenny is knocked unconscious, fractures her skull, breaks multiple ribs and both legs. Jenny is flown by Care Flight to the nearest Emergency Room where she is immediately admitted for surgery and spends a total of 7 days in an ICU room and another 15 days in the hospital. Luckily, in addition to her bronze level EHB health insurance, Jenny has a Level 4 Accident Direct plan. BILL After PPO Discounts Emergency Room Hospital Room (15 Days) $1,000 $5,000 $10,000 Paid By Health Plan $0 $6,650 Out of Pocket $1,000 $2,000 $3,350 The Bronze EHB plan only paid $6,650, leaving Jenny OOP $6,350. Accident Direct Renewability The policy is guaranteed renewable to age 65, subject to the Company’s right to discontinue or terminate the coverage as provided in the TERMINATION OF COVERAGE section of the policy. Eligible Dependents Under our business rules, Eligible Dependents covered under the policy may continue coverage until age 26, subject to the termination provisions of the policy.
