The end+disparities Learning Exchange a 9-month initiative that promotes the application of improvement interventions to reduce HIV-related disparities in four key subpopulations while building and sustaining a community of learners among Ryan White HIV/AIDS Program recipients. The Learning Exchange offers informational opportunities by content experts and utilizes the power of peer learning to facilitate local improvement efforts on eliminating HIV-related disparities. Michael

Communities of Learning: collaborative, QI campaign, Regional Groups Consultation: On/off-site coaching of recipients to advance their clinical quality management programs Together, we can make a difference in the lives of people with HIV. NQC provides assistance to RWHAP recipients to improve HIV care since gaps in HIV care still exist and advances are uneven across HIV populations. 250 recipients participated in 28 Regional Groups 51% of all recipients joined the largest HIV QI campaign; viral suppression increased from 70% to 76%, a statistically significant improvement Training and Educational Fora: monthly webinars, advanced trainings, online QI tutorials 40% of RWHAP recipients received TA and 95% would recommend TA to others 40 online QI tutorials are available; over 35,000 have been taken so far Intensity Information Dissemination: monthly newsletters, websites, publications, exhibits, QI awards Michael NQC is funded by the HRSA HIV/AIDS Bureau since 2004 and provides technical assistance to all RWHAP recipients to build their sustainable clinical quality management programs. Over 90% of all recipients have accessed intensive NQC offerings and take advantage of our various services. The Learning Exchange is an extension of these offerings. MSM of Color Voices in Healthcare Julia Introduce Amir and Joey Amir Simon is currently a student studying Human Services as well as a behavioral health specialist. For as long as he can remember, he has wanted to help people in any way that he could. Amir has always been a fighter for equal rights and an activist. After he was diagnosed 4 years ago with HIV, Amir was given a new outlook on life along with a deeper love and compassion for advocating for what he feels is right. Joey Pons began his activism in New York City in 1987 with ACT-UP the same year he was diagnosed with HIV. He was also co-founder of the organization Queer Nation and served as president of the National Latino Lesbian & Gay Organization (LLEGO). His career as an activist for the LGBT community is evidenced by having served on the boards of directors of 23 organizations. As a leader of the LGBT community he was invited to the White House twice during the presidency of Bill Clinton. In Puerto Rico, he has been co-founder of PR Community Network for Clinical Research on AIDS, the Sabana AIDS Civil Rights Litigation Project, Community Initiative and Foundation Article II. He has distinguished himself as a respected lobbyist.

Nathaniel Hendley Florida Department of Health Duval, FL Julia – introduce Nathaniel: Nathaniel, has been a part of the PLWHAA Community for nearly 35 years now. His history and Illness go back to when the disease was identified as GRID. After being diagnosed with full blown aids; 20 year of active substance abuse, and experiencing homelessness he is today a shining example of survival and hope. He is currently classified as undetectable; has 15 years of recovery; is a working professional, and is a very proud home owner. This long term survivor has seen and lived through the epidemic during a time when the disease was barely known to the community. He began his journey in an era when medications, treatment, and services were not available. In short, he and much of society were led to believe he was in the last stage of life with no real expectation of a future.   Nathaniel started his career working for the City of Jacksonville in the library system. After a year with the library he moved on to Barnett Bank, where he became a Computer Operator. There he set his sights on the military (Air Force). Since his military life, he has worked on various prominent jobs. Today, he is the Quality Improvement Manager for the Disease Control Programs with the Florida Department of Health Duval. Breakthroughs in HAART regiment have sustained his life, while recovery from substance abuse has given him a new outlook on life and hope for tomorrow.
