General Information What is the Hypertension Learning Collaborative? A shared learning environment with a mission to encourage programs targeting the improvement in the management of hypertension. Project teams will be able to regularly network with peers, participate in bi-monthly conference calls on relevant project topics, and have timely access to industry experts. Benefits $20,000 educational grant over a two-year period. Participation in learning collaborative. National recognition.

Project Timeline Collaborative Kick-Off Meeting: November 12-14, 2008, Scottsdale, AZ. Collaborative Start Date: November 30, 2008. Bi-Monthly Conference Calls: - Tuesday, January 13, 2009, 3:00-4:30pm (EST) 1st quarter progress reports presented by participants - Tuesday, March 10, 2009, 3:00-4:30pm (EST) Speaker to be scheduled  

Project Timeline Bi-Monthly Conference Call Schedule continued - Tuesday, May 12, 2009, 3:00-4:30pm (EST) 2nd quarter progress reports presented by participants - Tuesday, July 14, 2009, 3:00-4:30pm (EST) Speaker to be scheduled   - Tuesday, September 8, 2009, 3:00-4:30pm (EST) 3rd quarter progress reports presented by participants

Participants’ Responsibilities Implement project as outlined. Collect requisite quality and process metrics. Submit reports on time. Participate in all bi-monthly calls. Participate in face to face meetings. Share tools, insights, and protocols related to the project. Engage in open dialogue and discussion. Agree to allow AMGA and Daiichi Sankyo, Inc. to publish information such as interventions, findings, and other supporting materials related to the project. Submit the final repot to AMGA by October 2009.

AMGA’s Responsibilities Provide timely updates to participants. Be available for questions and/or concerns related to the project. Provide PowerPoint templates to participant to use for reports. Serve as resource and manage the collaborative. Maintain the collaborative website.

Kick-Off Meeting Purpose & Objective Opportunity to network with peers in the collaborative. Officially present your team’s project to the collaborative. Learn about other educational programs. Group Presentations AMGA will provide participants with a PowerPoint template to use. Participants will have 20 minutes to present and for Q&A. Kick-Off Meeting The Westin Kierland Resort & Spa Scottsdale, Arizona November 12- 14, 2008 Registration Email Danielle Flowers (dflowers@amga.org) to confirm by Wednesday, October 8, 2008. Travel Please plan to arrive on Wednesday, November 12, 2008 and depart Friday, November 14, 2008 no earlier than 2:00 p.m. Please provide Danielle Flowers with your flight arrival and departure dates/times by October 8, 2008.

Kick-Off Meeting Hotel Accommodations Make room reservations directly with The Westin hotel at (480) 624-1000 or (800) 937-8461. Reservations must be made by Friday, October 17, 2008 in order to receive the AMGA conference rate of $215 single/double. Check-in is at 4:00 p.m., check out is at 12:00 p.m. Special Note: If selected to receive a grant, travel and hotel expenses are to be covered by the educational grant . If not selected for funding, AMGA will cover coach airfare and ground transportation up to $700, and two night’s hotel for one person.
