Meeting Energizers… Company Challenge You as the champion need to explain what a meeting energizer is (hopefully starting this meeting with one) and encourage or challenge your group to incorporate meeting energizers into their regular day. Hint… You can use some of the energizers we have done at the Summit. Examples: Music, short activity that gets your blood pumping, quick game, a simple stretch routine, etc. Don’t forget to use your imagination. Send us pictures so we can share what works with your group You should fill in this Power Point Slide with your personalized group energizer

Behavior Change Board Long term “behavior change” starts with a clear commitment Declare your behavior change

Post your goal where you can see it Goal writing tips Should be clear (simple statement) Should be measurable (how will you know when you have accomplished your goal?) Should have a clear realistic time frame for completion Keep in mind a BIG goal is usually a series of smaller goals On your board Declare a goal you would like to accomplish LONG TERM What is at least one thing (To Quit/Add/or Replace) that would help you accomplish your long-term goal This year focus on these small changes that will lead to your big goal Your goal does not have to be weight related!!! Hint… Financial, Stress Management, Sleep, etc.

I Quit/I Add/I Replace

Examples… Lose 15lbs by April 1st Replace sugary drinks with water Quit going to the vending machine for snacks Add walking 3 times per week Save $1,000 emergency fund by March 1st Buy healthy snacks in bulk and bring my own (put that money toward emergency fund) Cut going out to eat by 1 time per week (put that toward emergency fund) Bring your lunch (put that toward emergency fund) Using the “I Quit/I Add/I Replace” Sheet is a way to hold yourself accountable to these small tangible actions that will lead you to your goal while creating sustainable habits

A Little Friendly Competition Each location is challenged to incorporate their “Behavior Change” board and goals throughout 2017 programming The location that does the best will be awarded “a location improvement” that contributes to wellness. *Subject to budget and will be reviewed upon that location winning Examples: Water fountains that fill water bottles and show the amount of recycled bottles you have saved by refilling a bottle Outfitting a standing meeting conference room Labeling a designated walking area with distance signs What can you come up with that your facility could/would use Judging will take place at the Summit based on the following criteria Creativity (using your boards and incorporating behavior change in your year) Outcomes (showing examples of your group/individuals working toward long term behavior change) Consistency and presence of the importance of long-term behavior change

Eligibility Requirements (Pg. 2) -2017 Wellness Program Enrollment Form – Most completed in November. -If you have not filled out a form and want to participate, now is your chance. -New Hire (Hired before April 1st 2017 must complete “All” requirements, however those hired after April 1st 2017 will be expected to do as much as they can if signed up for the program).   -Health Assessment (Must be completed March 1st – April 1st) -Health Assessments will be accepted NO LATER than April 1st -On-line or Paper form -Blood Work (Deadline May 1st) -Associate Blood Work April 1st – May 1st -(Any blood work completed and submitted to One Stop Wellness January 1st - May 1st, 2017 will be accepted for the Wellness Program). -Blood work can be submitted by e-mail onestopwellness@epbnet.com or fax to (888)-681-6146

Eligibility Requirements Continued (Pg. 2) -2 Qualifying Events (Deadline November 1st 2017) -Webinars -Lunch-n-learns -On-going Health Management Programs -(listed in the Qualifying Action Items) -Any approved programming for each individual site -A COMPLETE list of Qualifying Events are listed on Pg. 5 & 6 of your “Wired Up” Launch Packet.

Benefits Discount (PG 4) -2016 participation/completion = 30% discount for all of 2017 -2017 participation/completion = 30% discount for all of 2018 -Your discount will always be determined by the previous years participation/completion

Local “Wired Up” Activities Put your heads together for great ideas!!! Independent Programming Developed By Local Committees Is Encouraged

“Wired Up” Quarterly Agenda 1st Quarter (Pg. 3 & 4) 1st 10 Week On-going Health Management Program “Targeted… LDL” (January 16th – March 20th ) Take advantage of the opportunity to “Target” one of your most important numbers for cold hard cash. If your LDL is already in range, simply maintain this number. If it is out of range, simply improve it by the designated percentage (7%). You will be rewarded for either maintaining a healthy LDL or improving it by the designated amount (7%). Health Assessments (March 1st – April 1st 2017)

2nd Quarter (Pg. 3 & 4) -2nd 8 Week On-going Health Management Program “Summer Slim Down” (April 10th – May 29th ) Take the time to focus on dropping a few pounds before summer. This could simply be a continuation of the goals you’ve already been working on or a second chance to focus on your health. Lets work together to drop a few pounds while focusing on the power of protein. Associate Blood Work (April 1st – May 1st 2017) Spouse Blood Work – VOLUNTARY. For Details visit www.1stopwellness.net Click on the “Blood Work” tab under the SEWS tab for details). Spouses must register by March 1st in order to be eligible for free blood work.

Spouse Blood Work Voluntary (will not effect the benefits discount) Spouses only (No Children) Only spouses on SEWS insurance plan At a local LabCorp draw facility during the month of April for those who are registered Exception: Facilities that do not have a LabCorp facility reasonably close Participating spouses at these locations must follow the registration instructions below. ALL Spouses participating must register through One Stop Wellness Click on the “Blood Work” tab under the “SEWS” tab Click on the link for registration – Deadline to register = March 1st Once registered One Stop Wellness will mail your spouse instructions and a requisition form during the month of March. Results will be mailed to the address they designate within 7- 10 days after blood work is completed.

3rd Quarter (Pg. 3 & 4) 3rd 8 Week On-going Health Management Program “Counting Sheep” (June 12th – July 31st ) We take for granted how much a loss of sleep effects every aspect of our lives and health. More sleep can be a game changer for all your goals. Take the time to really focus on getting more sleep as well as improving the quality of the sleep you are getting Delivery of Results/Health Coaching (June 2017)

4th Quarter (Pg. 3 & 4) 4th 8 Week On-going Health Management Program “Get Infused” (August 8th – October 16th ) Have you ever wondered what all the fuss is about? Well, this is your opportunity to get infused. We’ll provide all kinds of recipes and ideas related to infusing your water. We’ll also take the time to educate you about the numerous health benefits. SEWS will provide water bottles to those participating (containing the times of day). Associates will be required to sign up and pay a portion of the bottle (if they want one). Associate responsibility will be $5. October – All about the Ladies… We’ll celebrate the ladies and focus on women’s health issues November – All about the Fellas… We’ll celebrate the men and focus on men’s health issues. November – “2” $1,000 Drawings. You get one entry for every non- webinar qualifying event completed. Deadline for Completing 2017 Action Items (November 1st 2017) Reenrollment into the 2018 Wellness Program
