Example 2: Summer only team with 3 coaches and one manager – 2 coaches and one manager will get full hours or coaches and managers can decide how they want to split the hours.

Player Paperwork MYSA Medical liability release form http://assets.ngin.com/attachments/document/0033/9172/medrelease.pdf Required for ALL MPB players regardless of league or age Complete each soccer year: Begins August 1 Fall 2016 form can be carried over to Summer 2017 Managers keep this form on file for the duration of the season.

Home Game Rescheduling Why 3 alternate dates? The Field Coordinator needs to fit your reschedule between games that are already scheduled, and with consideration to referee availability.  Providing more options ensures your reschedule can be completed as efficiently as possible. Can I request a specific time for my reschedule? You can request it, but it is not likely the request can be honored.    Reschedules need to be scheduled with consideration to both field & referee availability.  How do I reschedule an AWAY game? MPB Field Coordinator only reschedules MPB HOME games.  Please contact the coach/manager of the away team and they will facilitate the reschedule with their club contacts.

Game Reporting All MYSA teams must report game scores via Affinity Any games not reported incur a fine to both teams

Digital Player/Coach Pass Upload player/coach/manager photo onto your Affinity site. Bring your smart phone or tablet. Make sure to print your match report for each game, as cell service may be spotty at some fields.

Competitive Match Report Located on your Affinity team webpage Must print a new match report for each game. Match reports are available 4 days prior to the game. Once you print, you will not be able to edit that match report. Information will come from Player/Coach passes and game schedule received from MYSA. Plus player jersey numbers

After the Game Collect all items brought to field Corner Flags Confirm Game score on Affinity website Referees will report the score You need to confirm the score on your Affinity team webpage

MYSA Weather Policy If lightning is within five miles, with or without hearing thunder, the game(s) or practice(s) should be suspended and shelter sought.  (A rough guideline is to measure the time between the lightning flash and hearing the corresponding thunder. If it is 30 seconds or less, seek shelter.) Games are to be adjusted as the heat index rises: Mandatory water breaks Go to quarters Shorten the games In cold weather, players should be allowed to dress in appropriate clothing. MYSA Weather Policy: http://www.mnyouthsoccer.org/page/show/692946-mysa-weather-policy

Field Closures Field closures are determined by the home team/City MPB Field closures/weather cancellations can be found at the following link after 4PM on weekdays. http://www.maplebrooksoccer.com/page/show/282362-weather-status Contact opposing team if our fields are closed If no field closure or weather cancellation both teams must be at the field or game will be forfeited.
