Expertise applicable to eHealth related Research Patient is now at the centre of healthcare and should not be seen as a passive observer of their health needs Pharmacists with the use of technology will be able to manage medications and offer advice. In an open prescribing landscape, the medication team will be able to ‘see’ other prescribing and improve safety for the patient. With the advent of cloud technology, access to patient history across all health professionals reduces risk and shares the benefits for the patient.
Current eHealth related issues State/Federal – Hospital (LHD)/Medicare (PHN) NSW Health has invested heavily into an electronic system for managing their patient in hospital http://www.ehealth.nsw.gov.au/programs/clinical/emr-connect/emeds Federal government has continued funding for the Personal Health Record https://myhealthrecord.gov.au/internet/mhr/publishing.nsf/content/home Discharge of patients from acute care to community care – why is the communication so difficult? Governments are concerned that patients are representing to the acute system after being discharged - can medication reconciliation and the use of liason pharmacists redress the avoidable admissions associated with medication misadventure?
