Anchor updates Category 3 - TA Flags & Baseline Correction Form HHSC has recently developed a Category 3 Baseline Correction Form that providers submit following a TA session. If adjustments are requested, the form will be used instead of re-submitting the Category 3 baseline template that was used in October reporting. Expect to receive phone calls or emails on any TA flags from Jennifer, Noelle, or Amanda. NMIs HHSC will notify Providers of these results by March 31st If not approved, recoupment will occur in mid-April Anchor updates

Anchor Updates Waiver Renewal HHSC submitted the Waiver transition plan to CMS on March 22, 2015. Region 15’s diabetes learning collaborative was mentioned pg.4 Attachment A contains: Regional Tools to Share Information about DSRIP Projects Video Examples of DSRIP Projects and Collaboration Projects Reviewed on CMS Monthly Waiver Monitoring Calls Project Withdrawal Window - February 1st – May 1, 2015 No penalty for withdrawal during the withdrawal window. Any provider interested in withdrawing a project will need to timely submit a Project Removal Form to the waiver mailbox. Anchor Updates

Updated RHP Plans HHSC will post the Plans on the website mid-April: RHP Summary Information List of approved Category 1 & 2 DSRIP projects  - all RHPs (the list will continue to be updated as projects withdraw) QPI Summary – all RHPs (as of May 1, 2015). RHP Plan Information Most recent Category 1 & 2 Project Narratives by RHP Category 1 & 2 Project Workbooks for 3 and 4 year projects Category 3 – all RHPs Category 4 (to include RHP level summary data for PPE domains)   Updated RHP Plans

Each Anchor RHP website would have a “Stakeholder Resources” page, linked to /from HHSC site by May 15, 2015.  This would include information on stakeholder opportunities, original RHP plans (which would also remain on HHSC website) as well as a description of the process by which 3-year projects were added in that RHP: RHP Organization Original Executive Overview with potential addendum update Original Community Needs Assessment with potential addendum update for 3-year projects RHP certifications (these may be signed after all updated information from HHSC and RHP Anchors is available for review) – HHSC will provide a template for certifications in early March Also, the projects will be grouped by Project Type; Secondary Type; Medicaid, LIU, or Both: For Example, Behavioral Health and Integrated behavioral/physical healthcare for Both Payor Groups Updated RHP Plans

HHSC Mid-Point Assessment Meyers & Stauffer has started to review information reported for 3 year projects that did not have any information reported in April of 2014. Be aware that Meyers & Stauffer may contact providers with additional questions regarding any of the 3-year project that reported in October. Additional questions for 3 year projects will be coming in the near future. HHSC & Myers and Stauffer signed off on all Region 15 Change Requests and Plan Modifications April 1st, we will receive Myers and Stauffer results about the ‘reviews’ of our projects HHSC Mid-Point Assessment
