PRIMO project Proactive Risk Monitoring tool for Organisational learning Funded by Health Foundation 18 month project Based in SEAU Permanent frontline staff

Aims Encourage active staff participation Remove negative ideas of mistakes Elicit representative picture of local work environment Encourage active staff participation with clear feedback and learning

Questionnaire sent out to staff on SEAU 7 risk factor categories Each divided into 3 subcategories Extent on ability to work effectively is assessed Results assessed with MS Excel spreadsheet Collected data and comments totalled Most common hassles identified Extent of each effect on ability to work effectively is assessed

Questionnaire This is an example of one of the basic risk factors staff were asked questions on. A rating of 1 indicated that this was not an issue for them while working on SEAU, but a rating of 5 indicated a significant problem. There was also plenty of opportunity for staff to provide more specific answers to any of the questions asked. These are particularly useful in identifying areas for improvement.

Results for December

Initial results identified ‘easy wins’ Lack of equipment; drip, computers Poor communication eg NBM patients Action plan Funding for equipment Education sessions for staff F1 teaching sessions Update meetings with frontline staff Regular updates on notice board

Conclusion Implementation of project has led to Active staff participation in issues of clinical safety Improved communication skills Wider range of staff involvement Incorporate into regular risk assessment monitoring in all departments
