What is a scorecard? Measures performance against goals. Displays graphic indicators that visually convey progress and celebrate success of your council. These indicators provide a snapshot of council performance.

How is a council scorecard different from Intermountain’s Dashboards? A dashboard is made up of several scorecards or measures. Intermountain uses dashboards to follow all of their initiatives at a high level. A council scorecard measures success of specific council goals that may include Structure Processes Outcomes

Why a council scorecard? Enables the council to bridge the gap between strategy and actions Helps councils avoid ‘talking’ about problems but not accomplishing anything Visual reminder for all council members to stay focused on council goals

Where to start?...Be SMART What are the specific council goals? Are they SMART goals? Specific Measurable Action Oriented Realistic Time sensitive If the council goals are not SMART goals, re-write them in the SMART format.

Ideas for Structure Goals (used to measure basic council function) Number of meetings Attendance Presence of specific disciplines Minutes Agendas Time keeping

Example Your council struggles with cancelling meetings Goal: The council will meet on a monthly basis 10 times per year with July and December off. 2013 Council Meeting Schedule   Jan Feb Mar Apr May June Jul Aug Sept Oct Nov Date 15 Name Jane Smith 1 John Doe Dan Brown Total Attendance 10 Percent Attendance 62.5% Target Attendance 85% Progress Towards Goal -22.5%

Ideas for Process Goals (used to measure meeting effectiveness) Ground rules – do you have them and are they being followed? Problem/resolution in a timely manner Tracking issues so they don’t get dropped from the agenda Are all council members taking assignments? Are council members completing assignments?

Example: Issues keep getting dropped with no follow up Problem/Resolution Tracking Sheet 2013 Date Problem Description Action Items Assignments Comments Resolution 07/01/13 Increased patient falls Drill down falls by unit, falls with injury, type of falls, time of day and day of week Susie Jones to contact quality resources and bring data to the next meeting Aug meeting data reviewed and determined that falls are occuring in the bathroom because no railing is present Recommend to leadership and maintenance to add railing in all patient bathrooms 8/1/2013 Recommend railings to leadership and mainenance Take to coordination council in September Jim Brown to attend coordination council October meeting maintenance agreed to put railings on their schedule for 2014 Maintenance will put railings up in 2014. Need an interim solution for patient falls in bathroom 10/1/2013 Patients falling in the bathroom, railings will go up but we need an interim solution

Example: You want to increase compliance to bedside handoff report Bedside Report Tracking # of observations # of bedside reporting compliance Rate Goal Progress Towards Goal 12 6 50.0% 90% -40.0% 10 8 80.0% -10.0% 13 76.9% -13.1%  

You can talk about everything every time and get nowhere all the time OR you can talk about some things some of the time and get somewhere
