Scoring Form Circle the letter that responds to each question from your quiz. Count up the responses to the four specific working styles: Driver, Expressive, Amiable, and Analytical. Record at the bottom of this sheet.

What are your ratings? Again, this is not intended to be a comprehensive psychological evaluation. This is a friendly questionnaire for self reflection for your work place. The 5 Dysfunctions of a Team 1.) Absence of Trust 2.) Fear of Conflict 3.) Lack of Commitment 4.) Avoidance of Accountability 5.) Inattention to Results Members of a truly cohesive team behave in the following manner: 1.) They trust one another 2.) They engage in unfiltered conflict around ideas 3.) They commit to decisions and plans of action 4.) They hold one another accountable for delivering against those plans 5.) They focus on the achievement of collective results. With that in mind, Think about what you have learned with your work personality and the personalities of those around you, taking this concept of a truly cohesive team and create one SMART goal for yourself to take into the school year in working together as a team. Share out with your group.
