QFN is an end to end system that operationalizes quality.

The driving principles of QFN make quality improvement: Easy Profitable Professionally satisfying

Paradigm Shift Multiple levels Organization – Framework to organize the value of the ACC to promote its relevance to members and external partners Staff – Align divisional priorities to common Quality Vision Members – Providing the roadmap for accountability through quality and value

Quality First Network Mission To transform the delivery of cardiovascular care by providing a consistent and valuable demonstration of quality improvement, professionalism, accountability, and life long learning that will drive change in patient experience, practice viability, and reimbursement through a platform of services that will empower members and increase professional satisfaction.

QFN Goals Organizational Maximize value of the NCDR data to guide ACC quality vision Focus all ACC activities on common quality objective Direct all ACC education and training programs toward demonstrated quality outcome Provide practical vehicle to incorporate Quality First principles

Quality First Network Member Value and Practice Viability Position practices to advocate for payment models that reflect quality, equity, and value. Platform to influence national, regional, and local decision-makers Ensure ongoing professionalism Provide practice based tools to foster practices effectiveness and innovation

Sample Payer Subscription to QFN Level of Participation Access to QFN Fee Schedule ACC Branding/ Limitations Level I Agree to Quality Compact. Allow use of ACQL, PIMS and MOC to demonstrate quality. Allowed to use Self-Assessment Products to identify knowledge gaps. $1.00 pmpm Supporter of QFN. No Branding ability. Rules apply. Level II Utilizes CVRP Other QFN offerings might include: Hospital Recognition ACC CV Atlas Specified number of custom NCDR reports $2.00 pmpm Limited branding Level III Hospital Recognition Access to aggregated data from registries.
