Dementia Community Links Service This free service provides one to one support on a weekly basis to support people to live well with dementia in their own homes. The service enables individuals in West Lancashire with early stage memory problems or a Dementia diagnosis to maintain or re-develop daily living skills, have opportunities to be involved in educational, leisure or creative activities and be supported in maintaining or developing contacts in their local community. Delivered from a range of community venues in West Lancashire. Referrals can be made by friends/family, health professionals or self referrals on 0300 303 1234
Paid for services Home Help Support for older people in their home which includes, shopping, cleaning, small tasks. – Price is £14.50 per hour and referrals can be made via 0300 303 1234 Daytime Support – range of prices, half day, full day, lunch Support during the day for those who are physically frail, those with dementia or those who may be socially isolated.
