Claim Forms are Available: Provides a structure for emergency response. Incident Report forms should be completed as soon as possible after a claim producing accident or incident. Incident Reports should be completed as thoroughly as possible, and may include hospital or Drs. Statement Collision Reports for Auto Liability claims provide for a thorough review of incident and ability to transfer information. To consider a claim – notify the WNPS asap. Notify of the details of the accident. Download and complete forms and submit to the WNPS office. The office will work with the Broker to complete the claim.
