ED Portion Patient presents with chest pain complaint and you work them up like normal. Calculate a heart score (.procdoc) If score is 4 or greater admit to FIT. Place “erlanger 1 of 2” order set instead of transition order set If score is 3 or less we can send to the Low Risk Chest Pain Clinic IF the patient can exercise If patient cannot exercise, they are treated like 4 or greater
How to discharge Heart Score 3 or less In the discharge navigator you go to outpatient Rx/Referrals Place order for: ASA 81 MG Daily (unless allergic) Referral to Internal Med (REF 40) if no PMD May also refer to transition clinic (possibly only a temporary solution) Stress Test (Stress 118900)
How to discharge Heart Score 3 or less Ask HUC to call Centralized Scheduler to get appointment for patient in less than 72hrs. Clinic is open 3p-5p M-F. (soon HUC will be able to place on schedule by themselves- EPIC training pending) Stress test 253-573-7320 then 5, (730a-6p M-F) (45 min total) FMG 1-844-364-2778 (7a-6p M-F) Print heart score discharge instructions Clinic check in and registration is at DI (Walters Surgery Entrance)
