ACP Pilot goals #1 The high school staff will infuse ACP elements into each of the classes that they teach through rubric requirements for assessments and classroom activities Measure: 80% of teachers will share an example of an ACP connection with their colleagues at a staff meeting

ACP Pilot Goals #2 The structure of resource will enable students to work more closely with their teacher adviser Measure: 100% of students will meet with their advisers on a weekly basis. This will be documented using an Excel File to track elements of meeting discussions with each student.

ACP Pilot Goals #3 The ACP Leadership Team will collaboratively grow in their understanding of the process and distribute responsibility for the disseminations of information to all stakeholders Measure: 75% of the ACP Leadership Team members will attend monthly meetings at the local level and participate in statewide ACP development opportunities.
