Fig. 1 Classification of waste under Japanese law 224385 The actual state of home-care waste disposal at home-visit nursing stations in Japan: a descriptive questionnaire survey. Yukihiro Ikeda Kinki University Hospital Back ground Results In recent years, home-visit nursing care services have become widespread in Japan. Since the home-care waste resulting from home-visit nursing care is legally categorized as domestic waste (fig. 1). Municipalities are responsible for its disposal under Japanese waste management law. Table 1 Basic characteristics of the station Mean ± SE Opening year 1998.8±0.1 Full-time nurse 3.77±0.06 Part-time nurse 2.88±0.09 Number of visits of total a month 374.1±6.8 Industrial waste subject to special control Wastes Domestic waste Industrial waste Domestic waste subject to special control Fig. 1 Classification of waste under Japanese law Fig. 2 Do you carry the home care waste from patients' home to your station? Nurses at 1,308 stations (66.5%) responded to the survey. After excluding 26 stations that were closed and 5 stations that do not generate waste because their main care field is psychiatry. An analysis of the remaining 1,277 stations was made. The largest number of stations initiated in 1999. The mean number of full-time nurses and part-time nurses per station was 3.77 and 2.87, respectively, and the mean number of homes visited per station was 374/month. 79.3% of the nurses carried medical waste from the patients’ homes when they visited them. The most frequently observed supplies not carried were injection needles, tubes and bags. E-mail: yuyu@med.kindai.ac.jp Table 3 Do you have any trouble in carrying medical waste at home visits ? complaints Observation (%) I always have to carry the waste about with me after the first visit. 575 (45.0) I’m afraid of getting injured by the waste. 372 (29.1) The waste has an unpleasant odor. 355 (27.8) Table 2 Means of the visit Observation (%) Automobile 1138 (89.1) Bicycle 280 (21.9) Motorcycle 74 (5.8) Public transportation 20 (1.6) However, in fact, many municipalities do not accept the disposal of injection needles. Furthermore, regarding infusion bags, root, etc., which are generally thought not to be infectious. Visiting nurses carry back the waste that is not accepted by the municipalities, and the safety of such waste is left to the care of the nurses. The most frequently observed answer to the question, ‘Do you have any trouble in carrying medical waste at home visits?’ was ‘I always have to carry the waste about with me after the first visit’ (45.0%), followed by ‘I’m afraid of getting injured by the waste’ (29.1%) and ‘The waste has an unpleasant odor’ (27.8%). The means of transportation for the visits, the automobile (89.1%) and the bicycle (21.9%). Object I conducted a questionnaire survey with the aim of examining the problems involved in the disposal of infectious waste at home-visit nursing stations, in its handling during nursing-care home visits. It is necessary to provide the nurses with containers for medical waste suited to home-visit nursing care and tools for preventing injuries. Conclusion Methods From among the home-visit nursing stations registered at the National Association for Home-visit Nursing Care, 1,965 stations were selected at random, and questionnaires were sent to them.
