Manage Care –The project Active Ageing with Type 2 Diabetes as Model in the Development and Implementation of Innovative Chronic Care Management Models in Europe MANAGE-CARE focuses on the development of new chronic care management standards and guidance in Europe. The goal of the project is to develop an innovative Chronic Care Model with applicable standards for clinical pathways as well as guidelines and training curricular for healthcare professionals using these standards. MANAGE-CARE currently consists of 31 partners including all the leading organisations in Europe in the academic, research and public policy fields, mostly related to diabetes management. The project is led by Prof. Peter Schwarz of the University of Dresden and includes renowned partners such as Prof. Martin McKee of the European Centre on Health of Societies in Transition of the London School of Hygiene and Tropical Medicine.

Manage Care- THE GOAL Starting with diabetes as a chronic care example, MANAGE-CARE will develop an innovative chronic care model applicable not only for diabetes, but also for other chronic diseases. This model will adequately address the needs of elderly populations in particular, but also of young populations increasingly affected by chronic dis-ease, as well as challenges given by the changes in the healthcare systems and financing structures in European countries. The final deliverable of MANAGE-CARE will be a practical toolkit for the development of chronic care management programs applicable to health care management organization's, scientific and medical associations, but also insurance and payer stakeholders as well as the political partners. MANAGE-CARE will act as the basis for the development and implementation of innovative chronic care management programs in Europe. The results of MANAGE-CARE will be of relevance for all the stake-holders involved in diabetes care and management: healthcare authorities, healthcare professionals, professional organization's, the pharmaceutical industry, IT and health technology partners and political stakeholders.
