Goals for Continuity Site Patient Population (Urban Underserved) Peers Preceptors Patient Panel

Inpatient-Outpatient Blend 14 weeks total (FM plus IM) 2 week immersion in each inpt and outpt 10 weeks of inpatient with “cut outs” for outpatient care I Full Day per Week of Outpatient Continuity Clinic (Primary Care) ½ Day per Week Of Longitudinal Clinic (any specialty) ½ Day per Week for Home Visits, Patient F/U, etc

Continuity with Patients Panel of Clinic Patients (3 points of contact in 14 weeks) Outpatient Visits Urgent Care Inpatient Care Home Visits (include post-d/c) Phone Follow Up Specialty Visits

Results Clinical Skills Exam and Final Exam same as traditional model Focus Group: Continuity Clinic was a highlight because had more responsibility for pts and got to know them. Attendings resisted multiple days away from inpt but students and residents felt ok. 6 month continuity at one site allowed for greater learning, higher level of engagement, more significant role.
