OUTREACH The Gwent Experience Sally Copner Team Lead Critical Care Outreach Gwent Healthcare NHS Trust.

Service set up September 2001. Three stage approach. STAGE 1 – Awareness raising / cultural change. STAGE 2 – Outreach Education development. STAGE 3 – Outreach Team development.

Outreach Staff and current cover. Team Lead covering 5 days per week. 6 part time staff each covering 2 days per week. 9am – 5pm. Monday - Friday.

Work load. 2007. Follow Up’s – 748 Referrals – 1852 Admissions – 176 2008. Jan – Sept. Follow up’s – 709 Referrals – 1428 Admissions – 145.

Where are we know ? 24 wards with MEWS and full Outreach support. Outreach support on wards currently without MEWS as required. Introduction of SBAR Introduction and ongoing support of Care bundles at ward level.

Where are we now ? Facilitated discharge for patients with chest drains, CVC’s Tracheostomies. Audit and Evaluation of the Service. Links with resuscitation team regarding DNAR and Cardiac arrest calls. Follow up and respond to all clinical incidents associated with MEWS and referrals.

EDUCATION Working alongside ward staff and bedside teaching. Monthly Alert for all ward staff F1 education programme. Alert for all 3rd year student nurses at Cardiff in conjunction with Cardiff and Vale resuscitation service. Patient assessment teaching for all 1st and 3rd year student nurses at Uniglam.

EDUCATION. Observational Study Day for all Healthcare Assistants. Post registration teaching at University of Glamorgan and University of Wales Cardiff. In process of developing a new course c/o acutely ill ward patient.

What we did well ! Team members with appropriate knowledge skills and experience. Good relationships with all wards and all members of the multi disciplinary teams. NO WALLS !! Excellent Education Programme.

What next Awaiting Business case review. Aim to cover 8am – 8pm 7 days per week. Continuing to develop 24/7 with H@N. Back to basics.
