Chapter 10: Humans Consent: The Human Consent Form “But I did not want to do anything without your consent, so that any favor you do will be spontaneous and not forced.” Philemon 1:14

Introduction – IMPORTANT!! Humans were created in the image of God with the ability to make choices. God wants us to love him and have a relationship with him because we choose to. Real love comes by making a choice and following it up with a commitment. Humans have the ability to make a choice to participate in your experiment and then to commit to finishing it. All projects using human subjects must have subjects sign human consent forms.

Human Subject Consent Form! Following is a sample human consent form. You are welcome to copy it. -- Just change the areas where the red writing occurs to fit your own experiment. This form can also be found electronically on the http://www.questforeducationandarts.com/science-fair/ website so that it can easily be downloaded, copied, and changed

Sample Human Subject Consent Form… CHET – NW Science and Engineering Fair Human Subject Consent Form (Title of project) by (put your name here)   I am asking you to take part in my science fair research project. I am conducting a study that will determine whether people respond differently to music played in different keys and different modes. I am asking you to participate in this study to examine your responses to music because it is not clear if simply changing the mode and key of a piece of music is enough to change a person’s feelings. If you decide to participate, you will be asked to listen to a piece of music I have composed. I will play it for you in different keys. After listening, I will ask you fill out a survey after each key that will tell me how you feel about that piece. I believe the risks to this study are minimal and I will be careful to adjust the volume of the music so as not to hurt your hearing. You could possibly benefit from the results of this study by knowing if transposing a piece into a different key could improve your mood. Of course this will depend on the outcome of the study. Sample Human Subject Consent Form… All your study information, especially your name and contact information as well as your surveys will be kept confidential. I will not share this information with anyone except my parents, ____________________________ and my study advisors, ____________________________.   If you change your mind and do not want to continue in the study, you may stop at any time. However, I would ask that you allow me to keep the data we have collected so far. You must also return any supplies I have given you. There is no cost to you to participate in this study, but you will not be paid to participate. Any medical problems encounter from participating in this study will need to be covered by your current medical plan. I agree to participate and fill out the study surveys. I will also save all the study materials and return them to the investigator. This will include any unfinished surveys, CD and any other materials I may be given.
