CADCA 2017 GOT OUTCOMES! Coalition of Excellence Awards INSTRUCTIONS (Please read carefully): You will need to modify this template to provide CADCA with one Problem Overview slide for each substance use/abuse problem you are highlighting in your application. The “problem” will be in a RED box. Please assign each of the root causes you are sharing with their own colored box, and assign the associated local conditions you are sharing for EACH root cause to their own colored circle. Use basic statements (a brief sentence) to describe each the problem, root causes, and local conditions you shared in your Phase I application. You can pull these statements from the Problem Overview Worksheet you completed in step 1 of this application. Please do not include data on this template. This will serve as a cover page for the data and data sources you provide to back up your coalition’s problem analysis in your “Logic Model Snapshot” templates. Reminder: The problem, associated root causes, and local conditions you include here are those that your coalition has successfully addressed. Coalition of Excellence Awards INSTRUCTIONS (Please read carefully): You should have already completed one “Problem Overview” template for the substance use/abuse problem you are highlighting in your Phase II application before working on this template. Your “Problem Overview” slide(s) serve as a cover page for the information that will follow in these “Logic Model Snapshot” slides. You will need to complete one “Logic Model Snapshot” slide for each local condition identified on your “Problem Analysis Overview” (4 “Logic Model Snapshot” slides per substance abuse issue). To complete, first, transfer the problem statement, one root cause, and one associated local condition from your “Problem Overview” template to this “Logic Model Snapshot” template. Then, for each of the three logic model components, provide at least two specific pieces of baseline assessment data to support your statements. You should have already assembled the necessary data in the “Data/Strategy/Interventions Worksheet” completed in step 2 of this application. Be sure to include the year the data were collected and the name of the data source for each piece of data included in the “Logic Model Snapshot” slides. The data you include for each data source should link directly to the outcomes (improvements in your baseline data) that you present in your PowerPoint presentation. The baseline data are the specific numbers that confirm the existence of and relationships between the problem statement, root cause, and local condition. CADCA 2017 GOT OUTCOMES! Coalition of Excellence Awards INSTRUCTIONS (Please read carefully): You should have already completed one “Problem Overview” template and four “Logic Model Snapshot” templates for the substance use/abuse problem you are highlighting in your Phase II application before working on this template. Each “Strategies/Interventions Snapshot” slide should follow in the corresponding “Logic Model Snapshot” slide in your presentation. You will need to complete one “Strategies/Interventions Snapshot” slide for each local condition identified on your “Problem Overview” (4 “Strategies/Interventions Snapshot” slides per substance abuse issue). To complete, first, transfer the problem statement, one root cause, and one associated local condition from your “Problem Overview” template to this “Strategies/Interventions Snapshot” template. Then, provide at least five distinct interventions that address the local condition. You should have already described the required interventions in the “Data/Strategy/Interventions Worksheet” completed in step 2 of this application. Be sure to include whether the intervention is an individual-level prevention strategy or an environmental level prevention strategy. Then add a very brief description of the intervention (a more detailed description of the intervention can be made in the data/strategies/interventions worksheet completed in step 2 of the application instructions). If you need more room, continue on a second slide. The collection of strategies should comprehensively address the local condition and there should be minimal overlap between the interventions selected for each of the four local conditions.
