YEARBOOK AD ORDER FORM MUST BE FULLY COMPLETED AT THE VENDOR SITE Quantity and Total for AD Size and Price AD Copy: If they have instructions for what it should say BUT we really just want a business card or emailed image from them The VENDOR’S Signature and Date The last box requires you to fill out timely information (your name, ad photo and text received, etc.)

AD Contract This is a receipt Try to get cash or check while you are there Try to avoid the “BILL” option Fill out all info and get the VENDOR/CUSTOMER Signature White copy to vendor, yellow to YOU
