Key lessons learnt of community engagement and social mobilization in the Ebola response Dr. Glenn Laverack

October 2014-April 2015 Observations in the field and insights from the available literature. Identify key lessons of engagement and communication in the Ebola response. WHO, UNICEF and UNMEER.

1. The role of anthropology Anthropological insights can significantly contribute to Ebola control. Observed that ongoing anthropological work was not widely used in the Ebola response. In-depth and long-term inputs. Anthropology Vs Social science. The translation of anthropological findings into practical recommendations.

2. The communication approach Variation in communication approach across the 3 countries (SL:C4D, L:SBCC, G:CWC). Which combination would be most effective? SMAC Community Led Ebola Response (CLEA). Deep seated traditional practices. Self-management and community control. Using a bottom-up approach.

3. Community resistance Non-compliant behavior was a cycle of unwillingness to change traditional practices compounded by experiences of poor service delivery and weak information flow. Mistrust, fear and individual and community resistance. Observed that the nature of the resistance changed: less-more intensive. Mapping resistance and reticence.

4. Rumours Continuous flow of new rumours. Mistrust, fear, poor information flow and poor practice. Directly influenced programme delivery. Rumour identification, investigation and correction. How to prevent rumour and resistance? What specialist skills are required?

5. The urban context Urban and rural contexts present unique challenges. An alternative strategy to the ongoing rural –based approach was not fully developed. From where can we learn lessons? What are the requirements for an urban approach, for example, in densely populated slum and shanty areas?

6. Cross-border issues ‘Porous international borders’. Interwoven communities by language, ethnicity, traditions and economy. Increased foot and bicycle traffic acting as a potential source of infection. How best to manage: Is a systematic community management approach realistic? Off-limit areas.

Best practice in community engagement PRELIMINARY QUESTIONS   What is the purpose? Working definitions? Who is the community? Who are the stakeholders? Any existing groups? Cross-cultural team?   NEEDS ASSESSMENT Focus groups and participatory methods for mapping and prioritization. STAKEHOLDER CONNECTION Meetings with leaders, CAB, VHC, groups, representatives COMMUNICATION Awareness raising, technical information through appropriate channels.   INFORM THE WIDER COMMUNITY Participation = representation. Town hall meetings and information through appropriate channels. STRENGTHEN COMMUNITY CAPACITY   Facilitated dialogue, facilitators and participatory workshop. BUILD COLLABORATIONS AND PARTNERSHIPS   Experience sharing, seed funding, action across sectors and joint meetings. MONITOR AND FOLLOW-UP. FEEDBACK Meetings and information through appropriate channels.  

A phased approach The red phase indicates that community engagement has not yet been achieved. Personnel other than the community engagement team should not approach the community until this phase has been completed. The amber phase indicates that the community representatives have agreed to the programme to commence. The green phase indicates that the community representatives have agreed to allow all other personnel into or near to the community in order to implement the programme.

Ebola Care Group (ECG) members identified.   Cultural protocol Facilitated dialogue.   Updated district/county/ national situation FAQ and Fact Sheets Outputs Community leaders and members have agreed to move to the planning phase. Community representatives for the ECG have been identified. Broader PHC needs of the community has been identified.   AMBER PHASE Ebola Care Group (ECG) holds subsequent meeting(s) to agree on resources, location of ECU, number of beds, how to work together. Parallel community development group meets to discuss primary health care issues. Indicators of success identified. Outputs Location and operation of ECU agreed. Roles and responsibilities of the ECG identified. Training gaps and needs identified. Community development group established and key issues identified.   GREEN PHASE Resources and supplies delivered, ECU built, training completed, materials developed. Parallel community development group operational. Monitoring and Evaluation framework established. Outputs Trained people in place. Resources and supplies have been delivered. ECU established. Continuous feedback and monitoring mechanisms in place. Community receiving primary health care coverage.
