Spell things out in exacting details Example: The standard jack layout used by the BC IT Center is two Cat-6 RJ-45 jacks terminated for data, 1 Cat-6 RJ-45 jack terminated for voice and 1 RG6 threaded F-type coax jack. The standard faceplate will be four port single gang stainless steel. All voice connections shall terminate on beige RJ-45 eight (8) conductor CAT 6 modular jack assemblies. The voice jack will be placed in the bottom left the wall plate. Wire color-coding and termination shall be per EIA/TIA-568-B.2 specifications. All data connections shall terminate on yellow RJ-45 eight (8) conductor CAT 6 modular jack assemblies. The data jacks will be placed in the upper left and right ports of the wall plate. Wire color-coding and termination shall be per EIA/TIA-568-B.2 specifications. All video connections shall terminate on beige F-type threaded coax jack. The video jack will be placed in the bottom right of the wall plate. All pairs shall be terminated when connecting to voice and data jacks. Terminate as”568B”.

Searching key sections of the document turns up slight edits of the the U of L version on several other institution sites. Seems some version of this document has been scavenged by many others so its hard to say who originated it.
