Glaucoma Diagnostic Ability Of OCT Angiography Vessel Density Parameters Department of Ophthalmology, Konyang University, Kim’s Eye Hospital Jae Keun Chung, Young Hoon Hwang, Jae Min Wi, Mijin Kim, Jong Jin Jung, Hwang Ki Kim A Purpose To investigate the diagnostic abilities of the OCT-A vessel density (VD) parameters in the ONH, peripapillary, and macular regions for detecting different stages of glaucoma, especially in early stage To compare their diagnostic abilities to other OCT parameters, including RNFL thickness and GCC thickness Methods Retrospective, cross-sectional study A total of 113 healthy eyes and 140 POAG eyes were enrolled Glaucoma subgroup Early (MD > -6.0 dB) / Moderate (MD -6.0 ~ -12.0 dB) / Severe (MD < -12.0 dB) Diagnostic abilities of the OCT-A VD parameters in the ONH, peripapillary, and macula regions were evaluated by calculating the area under the receiver operation characteristic curves (AUCs) AUCs of the peripapillary vessel density parameters and circumpapillary RNFL thickness were compared Parameters ONH: inside disc VD (idVD) Peripapillary: peripapillary VD (ppVD) Macular: parafoveal VD (pfVD), foveal VD (fVD) VD of whole enface image: wdVD(disc scan), wmVD(macular scan) A 2012/05/09 B 3. <Diagnostic abilities of the VD parameters> Among the vessel density parameters, average peripapillary vessel density showed higher AUC than ONH and macula region (AUCs: 0.807, 0.566, and 0.651, respectively) for glaucoma detection SN ST N T IN IT <idVD> <ppVD> <pfVD & fVD> 4. <AUCs: ppVD vs. RNFL thickness> Results ppVD showed similar AUCs with corresponding sectoral RNFL thickness (P>0.05) However, in early stage of glaucoma, the AUCs of the IT and T ppVD were significantly lower than RNFL thickness (P< 0.05) 1. <Baseline characteristics> Discussion wdVD & ppVD had significantly higher AUCs than others wdVD and ppVD are parameters of the RPC layer Structural changes to the RPC networks has been implicated in the RGC axonal loss and RNFL change In the early glaucoma detection, the ppVD in the IT & T areas showed worse diagnostic ability than RNFL thickness Less prominent or temporally delayed changes in the VD than RNFL thickness at the early stage of glaucoma Decrease in VD may be a secondary change to RNFL loss OCT-A may have limitations in detecting minute vascular change in early stage glaucoma 2. <Comparisons of the VD parameters> OCT-A VD parameters as in ONH, peripapillary, and macular regions in glaucomatous eyes were significantly lower than healthy eyes (P < 0.05) A tendency of decreased VD with increased glaucoma severity was found Conclusion ppVD parameters showed similar glaucoma diagnostic ability with cpRNFL thickness, in early stage, VD parameters showed limited clinical value There is no commercial relationship relevant to the subject matter of presentation.
