Brooklyn Park ACH Brooklyn Park, a northwest suburb of the Twin Cities of Minneapolis and St. Paul, Minnesota, is an enriching community that celebrates diverse cultures, ages and economic status. We are building connections across organizations and agencies, partnering with families and neighbors to create a Community of Health where residents contribute their strengths, optimize their mental health, and increase social connectedness and healthy life styles.

Area(s) of Focus for our SCALE Work Brooklyn Park Clinic and NW Hennepin Family Service Collaborative Service Areas Medicaid-Eligible Families HCMC Integrated Health Partnership Enrollees Patients with a Diagnosed Mental Health Condition Train community members, both volunteers and staff of other community-based organizations, on topics such as leadership development, health equity, effecting local policy changes, basic preventive health and understanding mental illness. Connect health care providers and community partners, so that increased understanding of the cultural and systemic barriers to improved care can improve the provision of care and enhance residents’ healthful lifestyle. Develop plan to integrate health equity strategies into care coordination and population prevention strategies. Ensure active partnerships with organizations that can affect social determinants of health (such as housing, employment, education, safety, urban/suburban built and natural environment, and basic needs). Invite engagement through meetings, events, and outreach activities. Through one-to-one meetings, invitations to subsequent meetings and events, and other tactics, develop relationships. Ensure accountability to the local community by engaging and involving residents, including volunteer leaders from community-based groups, in decision making. Increase positive mental health outcomes Improve equity by pulling providers and resources together to create a continuum of referrals sources

HCMC Brooklyn Park clinic patients and their families with a mental health condition will be the defined population for this project, specifically targeting depression for enhanced care coordination and health improvement. Strategies determined by the Leadership Team and the Action Work Groups will serve to connect the clinic to the community for mental health and depression treatment services. As healthy lifestyles are the cornerstone of many health conditions, including mental health conditions, population based prevention strategies will focus on interventions that assist patients, families, and community members to be active, eat healthy, and be socially connected in the community. *Image borrowed from Minnesota Department of Health Accountable Communities for Health, 2014

We are most proud of our work to… Results of some of our past work together: Health promotion and prevention, within core clinical services and across community – clinical partnerships, include community-based screenings and delivery of wellness workshops where people live and work. Community outreach and engagement, include sending clinic staff to community-based organizations and networks, and inviting community members into CCT-developed programs and events, like Zumba in the parking lot of HCMC’s Brooklyn Park Clinic and walk and roll conversations and exercise with health care providers. Coordinated linkage among health services and social services includes standard work for care coordination focusing on transition from inpatient, emergency department, and HCMC’s acute psychiatric services back to primary care.
