System Issues Demand and current service provision for patients with challenging and complex mental health needs to deliver D2A Draft policy under development with clear pathway and process to compliment IDF for ratification w/c 06.11.2017 Helen Griffiths Number of failed discharges due to transport Co-ordinator will be in place within the IDT 23rd October. Monitoring daily supported discharges with Arriva Nikki Pownall Length of stay for Pathway 3 Escalated to commissioners for review Further clarity is required for Pathway 3 in relation to continuing healthcare ECIP to facilitate workshop with continuing healthcare on 20.10.2017 Amanda Kemp

Next Steps Long-term Actions Appointment to remaining agreed roles, including complex case workers, physiotherapy/ OT support to review community packages of care. Finalise IDF Standard Operating Procedure (SOP) and specification, including learning from early implementation using PDSA cycles and daily snagging lists. Implementation of workforce change for NUH employed IDT members to deliver 7 day coverage. The IDT will deliver a 7 day service and be a single point of access for the co-ordination and facilitation of transfer of care for all supported patients.   Long-term Actions Work to confirm financial governance around D2A model to support sustainable capacity changes across the system. Requires support from STP leads for enhanced financial visibility. From October – Leadership Academy to support IDF development. Nervecentre business case to support system wide adoption- implementation through 18/19.
