IMD deployed today and yesterday, and observed some minor sheening that appeared to be residual oil. IMD has opened the fund to ensure removal of pollution. As per the last inspection, the estimated pollution on board is APPROX 50 gallons of diesel and 25 gallons of lube oil. IMD intends to deploy again on 07 MAR to monitor the clean up contractors. IMD AST, D13 notified. PENDS On 08MAR17 the FOSCR and NPFC began setting up the claims process for local oyster fisheries that may be affected by the discharge. IMD personnel deployed to reassess the status of the sunken vessel and determined that light oil sheen was still present. The FOSCR contacted Global Diving and Salvage and will redeploy today to reassess the status of the sunken vessel and monitor the recovery of the remaining passive sorbent material by Global Diving and Salvage. A determination will be made on scene by the FOSCR to re-deploy fresh sorbent material if needed or secure operations. PENDS. On 09MAR17, IMD Astoria reported that on scene resources have removed 500 gallons of an oil/water mixture from the vessel's forward compartment. IMD located 02 fuel tanks via vessel schematic not previously identified with a max potential of 6000 gallons of diesel. IMD will re-deploy tomorrow morning to supervise further assessment of tank contents and removal of potential contaminants in the tanks. PENDS. On 10 MAR at 2030U, IMD Astoria returned to base and reported that they removed 500 gallons of oily water mixture from the vessel for a total of 1,070 gallons removed. They lined the deck with sorbent pads at amidships and the forward peak area. IMD intends to deploy again on Monday 13 MAR. PENDS. At 1135U on 13 MAR, IMD Astoria team returned to base. Global Diving & Salvage removed absorbent material placed on 10 MAR. IMD Astoria conducted an assessment and determined there was no recoverable oil remaining on the vessel. R/V HERO no longer deemed a substantial threat to the environment. Washington DOE will remain on scene and direct contractors to deploy and remove sorbent materials under their funding. Owens Kyle.M.Owens@uscg.mil (503) 861-6464 / 6477 Add Picture Add Picture At 1600T on 16 APR, SCR IMD received a call from WA State DOE regarding a vehicle in the water at the launch ramp in South Bend, WA. The owner was backing his 2014 Dodge Ram pickup with trailer down the ramp to retrieve his vessel. Upon exiting the vehicle, the truck and trailer continued down the ramp and into the Wilapa River. Local PD and FD responded to the scene and did not report any sheen from the submerged vehicle and were unable to locate and retrieve the vehicle. Locals also went at low tide on 17APR17 and could not locate vehicle. SCR IMD is working with local authorities and insurance company to put together a plan for removal, mitigation of a pollution discharge, and the possibility of a hazard to navigation. PENDS At 1554T on 17 APR, SCR confirmed that local resopnders were able to return to the scene at approximately 1230T in an attempt to locate the truck, but were unsuccessful. The responders however did note some bubbles and some sheening slightly upriver of the boat ramp. IMD Astoria is in contact with the responsible party and his insurance company who, at this time, intends to conduct salvage. SMIB issued. The owner also clarified that there was no trailer attached to the truck and that only his truck went into the water. At 1423T 18 April, IMD notified SCRCC that the owner and insurance company have deferred to the USCG for handling response of this pollution incident. IMD Astoria opened the fund and contracted with Global Diving and Salvage to find and remove the truck from the water in order to eliminate the threat of pollution. Global Diving will arrive on scene at approximately 1800T on 18 April to begin. Success truck salvage. 9/11/2018 FOSC Reports

Portland Yacht Club 18Apr RRT Activation: No Type and amount of product spilled: Est. 10 Gal Diesel Fuel Cause of spill: Marina Fuel dock Malfunction Date of spill: 18Apr Responsible Party: Portland Yacht Club Key operational activities: FOSCR directed marina personnel containment and clean up activities. Major lessons learned: None Lead Coordinator Contact Information: MST2 Sarah Augustine Sarah.M.Augustine@uscg.mil (503) 240-9370 At 1037T, SCR was notified of 10 gallons of diesel dumped into the Columbia River at the Portland Yacht Club. A fuel filter at the marina's diesel fuel dock failed resulting in 10 gallons of diesel dumped into the river. The source has been secured; boom and pads have been deployed to contain/pick up oil from the river. IMD PDX notified and deployed to investigate. PENDS At 1223T, IMD PDX notified SCRCC that IMD returned from investigating the diesel spill and determined the response from the Yacht Club was adequate; the Yacht club deployed absorbent boom and pads. The club will continue to monitor and replace the boom and pad as needed until it is cleaned up. IMD PDX will follow up in the morning with the Yacht Club to ensure it is cleaned up. 9/11/2018 FOSC Reports

Warehouse Fire, Willamette River 15 May RRT Activation: No Type and amount of product spilled: UNK amount of UNK products from fire fighting runoff into Willamette River Cause of spill: Warehouse Fire Date of spill: 15May Responsible Party: Stan Herman (Warehouse Owner) Key operational activities: NRCES was hired to place boom downriver to catch debris and wrap the warehouse in boom to contain any discharge/release. The water from fighting the fire is running off into the Willamette River near Freemont Bridge. Fire department has a boat on scene monitoring the situation. At 2228T, IMD PDX was notified and intends to follow up in the morning. IMD Portland confirmed the building owner is unable to provide boom in a timely fashion and the fire department is overwhelmed and unable to assist with boom. IMD Portland intends to open the fund and contract with NRC Environmental Services to deploy boom. No sheen is being reported by the fire department boat at this time but IMD cannot get eyes on the area due to the extreme danger of the fire. There is debris and presoak is coming off the pilings. The building is reported to have cars and marijuana inside. IMD PDX returned from deploying to the fire site. Determined no sheening occuring in the water and that the deployed boom was no longer necessary. The contract company cleaned up what fire debris that the boom had already collected and retrieved the boom. IMD no longer deems this incident a threat to oil/hazmat pollution. Expensive due to decontamination of PCB contaminated equipment and boiler. Remove tanks with product inside. NRC would utilize a crane to remove the containers on top of the barge and then disposal of tanks and contents off-site. BARGE 25 Primarily household HAZMAT. Goble Site Update: 06 June 2017 Estimated Totals: 02JUN17: 300 gallons of Bunker-C from the RIVER QUEEN's starboard fuel tank. Teams have collected, sorted, and packaged approximately 700 LBS of household hazardous material for disposal. 03JUN17: All totes on RIVER QUEEN were emptied, 10,000 gallons oily water mix discovered in AMAZON and removal began. 04JUN17: 14 propane and acetylene tanks were disposed of by flare team. Oil removal operations begin on tanks and bilge of Tug MANZANILLO. Contents of totes and barrels with waste oil that tested positive for elevated levels of PCBs were recovered into tank and kept separate from other waste streams. Department of State Lands officials inspected site today in preparation for transition of site control At conclusion of Coast Guard operations, Department of State Lands prepared to maintain control and security of site, and have begun initial staging for their response operations
