PHE 404 Undergraduate Internship

The Internship Process Choose a site Get approval from your advisor Complete the Learning Contract Add the credits with and ADD/DROP form Submit your summary report and evaluation

Choosing a site Start early! Find an organization in the community health field doing work that aligns with your goals. Contact the organization about a possible internship position. Visit our website for a list of potential organizations. Visit the PHE office (URBN 450) to look at the binder of internships recently completed by fellow students. Once you have an internship lined up, email Jane or Belinda for internship approval. Please include your Health Studies Concentration as well as a description of your internship responsibilities.

Resume and Cover Letter Most sites will want you to submit a resume and cover letter If you feel that you need assistance with writing either or both of these items, go to www.pdx.edu/advising-career-services Go to Quick Links and choose “The Career Development Job Readiness Guide. You will see Resumes and Writing Cover Letters as options. You can also call their dept at 725-4005.

Complete the Learning Contract This can be found on our website It is important to fill out the Learning Contract completely! Get your worksite supervisor’s signature

Add the credits with ADD/DROP form You cannot register online! You must: Bring in the email from your advisor approving your internship along with your learning contract Get your ADD/DROP form (available in URBN 450 or Neuberger Hall) stamped by advisor or office assistant by the end of the second week of classes Take your form to Neuberger (registration window)

Submit your summary report & evaluation Complete and submit: Student Summary, Supervisor Evaluation and Site Information page, which can be found in the internship packet on our website Your site supervisor will need to complete the evaluation and e-mail, mail or provide in sealed envelope.

Special Situations You can do your internship at multiple sites. For each new site, you must get approval and submit a new application You can pay for the credits over multiple terms. If you finish your internship in one term but pay for it in other terms, you need to email your advisor each term that you are registered to get the grade for that term

Responsibility Sites may require: Criminal Background Check, Proof of Immunizations, 1st Aid/CPR, etc. When at the site, you MUST: follow all policies, procedures and regulations; exhibit professional ethics, attitude and behavior; be punctual; and have absolutely spectacular communication skills with all staff including your supervisor!

Remember… Start early or you will miss deadlines! You can register for an internship as soon as you are allowed to register for your other classes Keep your advisors updated via email of any changes If you have questions please call or email 503-725-4401 schinfo@pdx.edu
