Line 1: Your name as it appears on your MySacState Student Center Line 2: Your 9 digit CSUS Student ID number Line 3: Your mailing address Line 4: Only list a secondary phone number if you have one. List an email address you check

Line 5: You are a Social Work major Line 6: Leave blank or write N/A Line 7: Year you began the program to year you plan to graduate. Full time students: 2016-18. Part time students: 2015-18. Line 8: Check yes if you plan to pass SWRK 250 Line 8A: Check GWI (Graduate Writing Intensive course: SWRK 250) Line 9: Write your practice instructor if they are full-time. If your practice instructor is part-time, choose any full time faculty or Dr. Torres. Leave Committee Members box blank or write N/A.

Only write in names of Instructors for classes you have completed/classes in which you are currently enrolled. Only write grades for courses you have completed.

Write Course #’s For Advanced Practice/Field/Policy courses assigned to your specialized area of practice. Write in Course #’s for Electives you believe you will take OR if you don’t know, write 299 (generic elective signifier).

Line 12: Choose whether you would like to do a thesis/project. MOST students do projects. Thesis vs. Project: a thesis requires two readers, a project only requires one reader Students can do a thesis or a project no matter which culminating experience class (500 or 501/502) they choose. Exam is not an option for MSWs. Table 12A: Line 1: Guess whether you will do SW 500 or SW 501. Circle one course number. If you Chose 500, Circle Thesis/Project in the ‘Title of Course’ line. If you chose 501, Circle Advanced Research Methods. Line 2: If you chose SW 500 in line one, Circle SW 500 and Thesis/Project again. If you chose SW 501 in line one, Circle 502 and Integrative Capstone Project. Leave instructor and grade boxes blank for both lines.

Line 15: Dr. Torres will sign after you turn in your Advancement Line 13: You sign and date Line 15: Dr. Torres will sign after you turn in your Advancement Once you submit your Advancement to the Division of Social Work, Dr. Torres will sign Line 15 and the SW staff will make three copies. The original will be kept in your student file in the SW Office and the three copies will be sent to OGS as required. The SW Office staff will not make you a copy of your Advancement. You should take a picture or make your own copy of the completed form before submitting as it is likely you will need to see your selections to make changes.

Make up to two changes per Petition. If you need to change your Advancement you will fill out a Petition for Exception. Changing or adding classes Taking a class during a different semester Switching from thesis to project or project to thesis Make up to two changes per Petition. Petitions must be turned in no later than 6 weeks before graduation. Petitions must have BOTH Faculty Advisor and Graduate Director signature before you submit to OGS.
